Systematic review and meta-analysis of endoscopic ultrasonography in staging diagnosis of esophageal cancer after neoadjuvant radiotherapy and chemotherapy

Abstract: Background: Clinically, it is necessary to evaluate the overall situation of the tumor before treatment, understand the disease stage of the patient, and choose the most reasonable treatment plan. Therefore, it is necessary to seek an efficient and accurate staging diagnosis method. EUS is widely used in staging esophageal cancer, but its accuracy will be affected by the experience of endoscopic diagnosis physicians.

“…PET/CT parameters are useful for predicting cancer prognosis 16 as well as checking current disease status. 17 Consistent with previous studies, follow-up PET/CT SUV values were helpful for the prediction of prognosis in terms of PFS and OS. [18][19][20] Previous studies have reported that SUV values of 2.5-5 after CCRT are adequate thresholds for prognosis.…”

“…Positron emission tomography and CT (PET/CT) are commonly used for tumor, node, and metastasis staging of malignancy. 18 F-Fluorodeoxyglucose is the most commonly used contrast agent and is a radiolabelled glucose analogue mainly concentrated in tissues with high glucose consumption, such as malignant tissues[ 23 ]. However, the assessment of regional lymph node status might be affected by the signal uptake from the adjacent tumor.…”

“…EUS is regarded as a more accurate tool for preoperative local staging of esophageal cancer [17]. However, the efficacy of EUS in terms of determining an individual patient's T category is inconsistent, and this method is even more deficient for assessing those receiving neoadjuvant CRT [18]. The fluorodeoxyglucose (FDG) avidity of the primary esophageal tumor is significantly positively correlated with pathological T-stage [19,20].…”
