Systematic review and meta-analysis of the effects of statin therapy on abdominal aortic aneurysms

Abstract: The claim of a reduction in AAA expansion rate with statin therapy is based on low-quality evidence and was not significant on meta-analysis. However, statin therapy did appear to improve all-cause survival after AAA repair.

“…The findings reported here for lipid-lowering drugs are consistent with other meta-analyses showing a possible association between statin usage and slower aneurysm growth rate. 18,19,98 The finding that ACE inhibitors increased growth rates in the UKSAT 79 was not substantiated by other studies. Particularly in the older studies, few of the patients included in the current meta-analysis were taking cardioprotective medications and even if patients were taking these drugs at baseline, there is no evidence about continued patient compliance with medication.…”

“…Statins should be prescribed as they decrease all---cause mortality and cardiovascular mortality after surgery for AAA (Kertai et al 2004) and also reduce the risk of cardiac morbidity and mortality by half, within 30 days of surgery (Schouten et al 2009). Previously it has been thought that statins would reduce growth---rate of AAA (Schouten et al 2006, Schlösser et al 2008, but a more recent and larger study indicates that this is not the fact (Ferguson et al 2010), and the same conclusion is drawn in a meta---analysis (Twine, Williams 2011).…”

“…Due to current investigation of markers associated with AAA, targeted ultrasound imaging may be a future option (Moxon et al, 2010;Villanueva 2008). With increased knowledge of pathophysiological mechanisms, pharmacotherapy or gene therapy may be available for stabilization of aneurysms (Baxter et al, 2008;Cooper et al, 2009;Golledge et al, 2009;Raffetto & Khalil, 2008;Twine & Williams, 2011). It may then be interesting to apply drug-or gene-loaded contrast agents, which could be monitored and destructed using ultrasound for local drug delivery.…”
