Systematic Review and Meta-analysis of Therapeutic Management of Upper-Limb Dysfunction in Children With Congenital Hemiplegia

Abstract: No one treatment approach seems to be superior; however, injections of botulinum toxin A provide a supplementary benefit to a variety of upper-limb-training approaches. Additional research is needed to justify more-intensive approaches such as constraint-induced movement therapy and hand-arm bimanual intensive training.

“…Our previous systematic review and meta-analysis identified 12 randomized controlled trials (RCTs) of constraint-induced movement therapy (CIMT), hand arm intensive bimanual training (HABIT), neurodevelopmental treatment (NDT), and intramuscular injections of botulinum toxin A (BoNT-A) augmenting occupational therapy (OT). 3 Findings suggested that intramuscular injections of BoNT-A provided a modest supplementary effect to OT on improving UL outcomes and a strong effect on improving individualized goals. The limited studies of NDT indicated weak to moderate effects on improving quality of UL movement and fine motor skills, despite being commonly used in clinical practice.…”

“…Adequately powered RCTs of CIMT and HABIT using reliable and valid outcome measures were recommended. 3 In the past 4 years, a large number of RCTs particularly investigating CIMT and modified CIMT (mCIMT) have emerged.…”

“…Ботулинотерапия -часть восстановительного лечения ребенка с ДЦП, что неоднократно подчер-кивалось в консенсусах по лечению таких больных [3,10,11], а также в нескольких обзорах и метаана-лизах [2,12]. Этот метод входит в число основных лечебных подходов (наряду с функциональными методами терапии и консервативным ортопедиче-ским лечением), которые применяются у большин-ства пациентов со спастическими формами ДЦП [3].…”

“…The results of these exams are important to determine the patient general condition, and they allow doctors prescribing drugs to control the recurrent seizures, relax the muscles, or conducting surgeries of different kinds (e.g. some patients require tubes feeding or the cutting of certain nerves of the spinal cord to reduce the pain and spasticity) [8,9,10].On the other hand, it is fundamental for patients with CP to receive co mp lementary treat ment activities at home that can with her/his healthcare. For example, some patients require good nutrition and intestinal protection with softening of feces, fluids, fiber and laxat ives in addit ion protection against injuries to their joints [9,10].The prenatal care reduces significantly the risk of suffering CP in children.…”

In the most of cases, when a wo man presents complications during pregnancy and is treated in time at a prenatal clinic, is possible to prevent lesions that cause disorders in the brain of the fetus [11,12].…”
