Systematic Literature Review of Treatment Options and Clinical Outcomes for Patients With Higher-Risk Myelodysplastic Syndromes and Chronic Myelomonocytic Leukemia

Abstract: High-dose chemotherapy with allogeneic hematopoietic stem cell transplantation (allo-HSCT) can produce long-term remission in patients with higher-risk myelodysplastic syndromes (HR-MDS) and chronic myelomonocytic leukemia (CMML). However, this treatment regimen is not appropriate for elderly and/or comorbid patients; in these cases, azacitidine is a standard treatment. This systematic review was conducted to evaluate real-world evidence of treatment options for patients with HR-MDS/CMML. Medline and Embase (J… Show more

“…A variety of different “classical” therapeutic strategies have been used to eliminate cancer or stop its development. For many cancer types, such as some hematologic tumors, chemotherapy is the most effective treatment ( Mousavi et al, 2016 ; Bell et al, 2018 ). For other types, surgery is the best option ( Marcasciano et al, 2017 ; Ye et al, 2017 ); when surgery is not possible, chemotherapy can be helpful in stopping tumor growth or reducing its volume ( Wagner et al, 2017 ; Wang et al, 2018 ).…”

“…Given the extensive epigenomic aberrations seen in young patients, it is possible that these neoantigens may be epigenetically silenced. This, in turn, would lead to poor prognosis even though the patients are enriched for immune pathways, potentially explaining the increased sensitivity of epigenetic drugs in younger patients(Bell et al, 2018). While the datasets used for this analysis did not have a substantial amount of triple negative cases, future population-based studies could test this hypothesis.…”
