Systematic comparison of the effectiveness of percutaneous mitral balloon valvotomy with surgical mitral commissurotomy

Abstract: Clinical evidence suggests there are comparable clinical outcomes in terms of operative, late mortality and complications between percutaneous balloon and surgical mitral commissurotomy. Incidences of new-onset mitral regurgitation and late re-intervention are significantly higher after balloon intervention.

“…5,6 Some studies comparing PTMC with surgical commissurotomy (SC) have shown that its incidence of complication in long-term follow-up was similar in both procedures. 7,8 The success and safety of the PTMC is dependent on patient selection. Some factors, such as patient age, functional class, previous commissurotomy, pre-procedure mitral valve area (MVA), and valve anatomy all predict the outcomes of this procedure.…”

“…9 Although the evidence is not convincing regarding the superiority of either PTMC or SC, because of the greater morbidity associated with open surgery, the PTMC is a preferred procedure in young patients with favorable valve morphology. 8 Patients with lower Wilkins score benefit more from the PTMC than surgery and have better clinical outcomes. 10 Although there is sufficient evidence to support PTMC in the lower Wilkins score (≤8), 10 there is not sufficient evidence to support PTMC in the higher Wilkins score (>8).…”
