System-related interventions to reduce diagnostic errors: a narrative review

Abstract: Background Diagnostic errors (missed, delayed, or wrong diagnosis) have gained recent attention and are associated with significant preventable morbidity and mortality. We reviewed the recent literature to identify interventions that have been, or could be, implemented to address systems-related factors that contribute directly to diagnostic error. Methods We conducted a comprehensive search using multiple search strategies. We first identified candidate articles in English between 2000 and 2009 from a PubMe… Show more

“…Contributors to diagnostic error include gaps in provider knowledge or memory (e.g., the ability to recall a similar case), mistakes in judgment, fatigue, poor referral choices, incomplete history-taking, misinterpretation of laboratory tests, and provider overconfidence [36,[38][39][40][41][42]. Cognitive [43] and system-related interventions [44] to reduce the likelihood of diagnostic errors have also been examined.…”

“…Diagnostic error can take several forms, including missed, delayed, or incorrect diagnosis [5]. Interventions to reduce diagnostic error include system-level and cognitive strategies [6]. A complementary approach is to focus on individual steps in the diagnostic process as well as the diagnostic process as a whole to identify sources of error and areas for improvement.…”

“…Decisions can be strongly influenced by others involved in or impacted by the decision. A review of interventions that aimed to reduce diagnostic error suggested that controlled settings and study methods often neglect contextual influences on clinical decisions [17]. Different care providers may have different thresholds for action which may result in decisional conflict.…”
