The majority of abnormal findings or lesions on T2-weighted fast spin-echo (FSE) magnetic resonance imaging (MRI) are hyperintense due to increased perfusion or fluid content, such as infections, tumours or synovitis. Hypointense lesions on T2-weighted images (both fat-suppressed and non-fat-suppressed) are less common and can sometimes be overlooked. Such lesions have limited differential diagnostic possibilities, and include vacuum phenomenon, loose body, tenosynovial giant cell tumour, rheumatoid arthritis, haemochromatosis, gout, amyloid, chondrocalcinosis, hydroxyapetite deposition disease, lipoma arborescens, arthrofibrosis and iatrogenic lesions. These lesions often show characteristic appearances and predilections in the knee. In this article, the authors describe the MRI features of hypointense T2 lesions on routine knee MRI and outline a systematic diagnostic approach towards their evaluation. Key Points • Hypointense lesions on T2 images (T2 Dark Lesions) encompass limited diagnostic possibilities. • T2 Dark lesions often show characteristic appearances and predilections in the knee. • A systematic diagnostic approach will help radiologists make the correct diagnosis.

GirişSinovyum lokomotor sistemde eklem, bursa ve tendon kılıflarını döşeyen özelleşmiş bir mezenkimal dokudur. Sinovyal eklemler iki tabaka ile çevrilidir; yüzeyde kalın bir fibröz kapsül ve içe-ride daha ince olan sinovyal membran. Normal sinovyum iki tabakadan oluşur; sinovyal intima ve subsinovyal doku. Subsinovyum vasküler ve kapiller bir ağ içerir. Bu kapiller ağ sayesinde plazmadan süzülen sıvı ekleme ulaşır. Diz eklemindeki sinovyal membran insan vücudundaki en geniş ve kompleks olandır. Diz ekleminde sinovyal membran femur, tibia ve patellanın artiküler yüzeylerini, eklem ile ilişkili resesleri, menisküs ve intraartiküler bağları döşeyerek, eklem ile ilişkili çok sayıda bursaya uzanır.Enflamatuar, enfeksiyöz, dejeneratif, travmatik ya da neoplastik kökenli olabilen sinovyal patolojiler sıklıkla diz eklemini etkiler. Sinovit eklemi döşeyen sinovyum tabakasının irritasyon ve enflamasyonudur. Sinovyal membran hasarlanmaya sıvı üreterek yanıt verir. Böylece sinovitin tipik klinik bulguları olan şişlik, ağrı ve kızarıklık ortaya çıkar. Bu durum enfeksiyon, kanama, romatolojik hastalıklara bağlı olabileceği gibi pigmente villonodüler sinovit (PVNS) hemofili ve sinovyal hemanjiomalarda olduğu gibi hemosiderotik özellikte, sinovyal kondromatozis veya lipoma arboresans gibi patolojilerde, kristal artropatilerde ya da yabancı cisimlere reaktif olarak görülebilir.Dizi oluşturan kemik yapılar ve olası kalsifiye patolojilerin değerlendirilebilmesi amacıy-la başlangıç inceleme yöntemi direkt grafiler olmakla birlikte, sinovyal patolojinin değer-lendirilmesinde seçilen görüntüleme yöntemi MRG'dir.Diz eklemi anatomik olarak en geniş sinovyal membrana sahiptir ve MRG incelemelerinde sıklıkla sinovyal patoloji gözlenir. Klasik radyolojik bulgular efüzyon ve sinovyal kalın-laşmadır. Buna eşlik edebilecek diğer bulgular spesifik bir tanıya yönlendirebilir. Sinovyal patolojilerin ayırıcı tanısı, diz eklemindeki tutulum alanı, farklı sekanslardaki sinyal özellik-leri ve eşlikçi bulgular değerlendirilerek yapılır ve böylece girişimsel işlemlere olan gereksinim azaltılır.Sinovyumu görüntülemede MRG'nin üstünlü-ğü kesin kabul görmekle birlikte, özellikle kontrast madde kullanımı hakkında farklı görüşler bulunmaktadır. Background Sarcomas in or contaminating the knee are rare but extremely challenging to treat. Complete resection of the joint is necessary, and often the entire extensor mechanism is removed as well. Reconstruction of the knee is challenging, and the resulting function may be compromised. Description of technique We describe a surgical technique of extraarticular resection of the knee while preserving the extensor mechanism combined with prosthetic reconstruction. The medial and lateral retinaculum is prepared such that it allows extraarticular placement of K-wires that are driven through the patella and the proximal tibia, serving as in situ guides for the osteotomies. Patients and Methods We retrospectively reviewed 11 patients with sarcomas contaminating the knee. The minimum followup was 14 months (mean, 38 months; range, 14-80 months). Results At last followup patients had a mean flexion of 88°(range, 65°-120°). We observed no complications related to the extensor mechanism, and there was one local recurrence.
