Synostotic Frontal Plagiocephaly: Anthropometric Comparison of Three Techniques for Surgical Correction

Abstract: Surgical correction of synostotic frontal plagiocephaly ("unilateral coronal synostosis") focuses on distortions of the forehead and orbits. Technical variations include unilateral versus bilateral fronto-orbital positioning. Surgical alignment of the deviated nasal root was introduced in our unit. Anthropometry was used to assess anatomic outcome, and results were compared in 22 children with synostotic frontal plagiocephaly who had either (1) unilateral fronto-orbital advancement ("canthal advancement") (n =… Show more

“…At present, anterior plagiocephaly represents a great challenge for the craniofacial surgeon 2,14,20,21,29 . We advise to use the conventional unilateral fronto-orbital advancement with "tongue-in-groove" design in the temporal region combined with supraorbital "bandeau" in severe cases and to include nasal osteotomies if neccessary.…”

“…On the contrary, anterior plagiocephaly has always been more laborious to correct, being complications more frequent and its results difficult to predict 2,14,20,21,39 . Nevertheless, there were few complications among our patients and we did not find significant differences between the two techniques performed (Type VII: frontal bilateral remodelling without frontal-orbital "bandeau" and Type VIII: frontal bilateral remodelling with frontal-orbital "bandeau").…”

“…Hansen et al 11 compared the outcomes achieved with 3 different surgical techniques in the management of unicoronal plagiocephaly repair. Their report suggested that only surgical techniques that included the osteotomy of the nasal root significantly improved the nasal canthal associated with the C-shaped deformity of the face.…”

“…3 Two-dimensional photography has been widely used and validated to evaluate the characteristics of craniofacial malformations and to assess the effect of therapeutic surgical procedures in the correction of such conditions. 1,3,10,11,22,32 Three-dimensional photography is a novel technology that allows us to capture a patient's superficial craniofacial anatomy in 3 dimensions. 14,19 This revolutionary imaging technology has the potential of quantifying volumetric changes of the facial soft tissues 6,16,19 as well as cranial vault volume.…”
