Background: This study had two objectives: firstly, to describe and compare sublocations of all cancer of the oral cavity, oropharynx, hypopharynx, larynx and primary carcinoma cervical lymph node diagnosed in Reunion Island, a tropical French overseas territory in the southern hemisphere between 2009 to 2013; and secondly others epidemiological features.Methods: A retrospective study included 621 patients diagnosed with cancer of the oral cavity, oropharynx, hypopharynx or larynx or primary cervical node between 2009 and 2013 in Reunion Island. 13 possible sublocations of cancer for mouth are described, 14 for larynx, 12 for oropharynx, 3 for hypopharynx, 8 for primary cervical lymph node. Demographic characteristics, data on alcohol consumption, smoking habits, HPV infection, denutrition were analyzed. Results: Cancer location consisted of oropharynx (36.2%), larynx (25.6%), oral cavity (20.8%), hypopharynx (13.8%), primary carcinoma lymph node (3.6%). Sublocations in oral cavity mainly concerned palatin tonsil and base tongue. Vocal fold was the mostly frequent sublocation involved in larynx. Cancer in oral cavity were portion out more homogeneously. Excluding primary lymph node, sex ratio was 7.7 and mean age was 60 years. Cancer consisted of squamous cell carcinoma in 99.1% of patients. 340/375 patients (81.25%) showed alcohol abuse; 309/359 (86.1%) were smokers; 31/184 (16.8%) had HPV infection.Conclusions: First study including all cases of pharyngolarynx and oral cavity cancer and depicting all sublocations involved in one series. We found no significant difference of distribution between sublocations for larynx but some for oropharynx and oral cavity compared to literature.

INTRODUCTIONThe war against cancer which started long back is still continued by the oncologists and radiotherapists. Despite the various modalities of treatments offered, the cure rates are not much gratifying. Although this is the situation regarding a single malignancy, now a days we encounter patients with double and triple malignancies. As a result of improvement in medical facilities and thereby the survival of cancer patients, the incidence of double malignancies is on the rise.1 The second cancers not only add to the morbidity and mortality of the patients but also to the distress of the treating surgeon and hence there is a need to study the nature of these double malignancies. The objective of this study was to estimate the frequency of double malignancies among all the malignancy cases studied, to analyze the common types of malignancies that occur together in relation to the ABSTRACT Background: The occurrence of second malignancies in cancer survivors is not uncommon. Due to advances in diagnostic and therapeutic modalities, the survival of cancer patients has improved and the incidence of second malignancies is also on the rise. The second malignancy may be synchronous or metachronous. Though there are plenty of case reports on double malignancies, original studies are less. The present study aims to analyse the frequency, types of double malignancies and their effect on the morbidity and mortality of the patients. Methods: This is a retrospective and prospective analysis of cancer patients done in a tertiary care hospital. All patients who presented with double malignancies from January 2004 to December 2014 were included. Retrospective data was obtained for cases from January 2004 to October 2011 and prospective data was obtained from November 2011 to December 2014. The study included 32 patients with double malignancies. Results: Gynaecological malignancies were most common in our study, followed by head and neck and gastrointestinal malignancies. Most of the malignancies were synchronous (56.25%). Double malignancies involving the head and neck, female reproductive tract, breast can be explained by common etiological factors like Human papillomma virus (HPV). Field cancerisation explains the synchronous cancers in Head and neck, upper aero digestive tract and reproductive tract. Conclusions: This study emphasises the importance of screening cancer patients for other primary malignancies during initial evaluation as well as during follow up period, in addition to the metastatic workup. It also stresses that timely reduction of risk factors in cancer survivors can prevent further malignancies.

Response to R-CHOP in HPV-related squamous cell carcinoma of base of tongue: a case report

Background: Synchronous squamous cell carcinoma of the head and neck (HNSCC) and non-Hodgkin's lymphoma is a rare clinical scenario. It is unknown whether the R-CHOP chemotherapy for lymphoma would also be active against HNSCC. Herein, we present such a case and a review of the literature. Case presentation: A 64 year-old female presented with painless jaundice. CT demonstrated a retroperitoneal mass and pathology showed follicular lymphoma. A base-of-tongue HPV + squamous cell carcinoma was found incidentally on staging CT. R-CHOP chemotherapy was initiated. After 3 cycles of R-CHOP the lymphoma had a complete metabolic response and, unexpectedly, the HNSCC also demonstrated excellent response. The patient received another 3 cycles followed by radiation to the HNSCC and to date is in remission for both cancers. Conclusions: This case highlights the exquisite sensitivity of HPV-related HNSCC, which should be taken into consideration in treatment prioritization of a concurrent diagnosis of a second cancer.
