Symptomatic Pneumocephalus after Lumbar Disc Surgery: a Case Report

Abstract: Symptomatic pneumocephalus is frequently seen after traumatic fracture of the skull base bone. However, it has rarely been reported after spinal surgery and its mechanism has not been fully explained. In this paper, we present a 30 year old male patient who had lumbar discectomy due to a symptomatic midline lumbar disc herniation. He had developed symptomatic pneumocephalus after the lumbar disc surgery associated with application of a vacuum suction device. We present and discuss our patient in the light of t… Show more

“…In the same study, it was thought that when the vacuum suction device was placed on the fascia after spinal surgeries, it could be a predisposing factor in the development of pneumocephalus. 11) Apart from these surgical cases, pneumocephalus was observed in a patient who developed symptoms mimicking stroke after lomber epidural steroid injection. Pneumocephalus is a well-known complication of spinal and epidural anesthesia, but it is very rare after lumbar epidural steroid injection.…”

“…The first involves the “inverted bottle theory,” where cerebrospinal fluid (CSF) leaks out creating negative pressure in the subarachnoid space allowing air to enter. 6 7 8 9 10 The second theory is described as a “ball valve mechanism,” where a dural tear resulting from a fracture allows air to enter but cannot exit, similar to a tension pneumothorax. 6 7 8 9 10 …”
