Symptom Cluster Experience Profiles in Adult Survivors of Childhood Cancers

Abstract: Although aggressive medical treatment protocols have led to 80% 5-year survival rates for most childhood cancers, many long-term survivors experience multiple troubling symptoms. Using data from 100 adult survivors of childhood cancers (ACC-survivors), we used latent variable mixture modeling to generate unique subgroups of survivors based on their experiences with a cluster of eight symptoms: lack of energy, worry, pain, difficulty sleeping, feeling irritable, feeling nervous, difficulty concentrating, and fe… Show more

“…This cluster of physical health, pain, and emotional concerns has previously been observed [8, 9, 22, 23]. Poor survivor-reported health was significantly associated with SI even after controlling for demographics, diagnosis and treatment factors, and depression.…”

“…16,17 Scant research supports the association between symptom prevalence and HRQOL in long-term survivors of childhood cancer. 5,8,18 The Childhood Cancer Survivor Study (CCSS) observed that survivors who reported more fatigue, sleep disturbance, and daytime sleepiness also reported greater HRQOL impairment than survivors with fewer symptoms. 8 These studies emphasize the effect of individual symptoms on HRQOL rather than the effect of combined symptoms as a whole, leading to underestimating the overall impact of symptoms and limiting our ability to compare the relative contribution of individual symptom to HRQOL.…”

“…Third, the symptoms under investigation did not include fatigue and sleep disturbance, which are deemed important to survivors. 5,6 Therefore, the extent that these symptoms are associated with HRQOL and the variance in HRQOL explained by the symptoms might be underestimated.…”

“…Among ACC-survivors from the CCSS, 10%-23% reported moderate to extreme pain (Hudson et al, 2003; Lu, Tsao, Leisenring, Robison, & Zeltzer, 2007);16%-40% significant fatigue (Hudson et al, 2003; Mulrooney et al, 2003; Mulrooney et al, 2008); 12%-16% problems sleeping (Mulrooney et al, 2003; Mulrooney et al, 2008); 5%-17% psychological distress (i.e., anxiety, depression, somatic distress) (Zeltzer et al, 2009); and 14% difficulty concentrating (Ness et al, 2008). In a previous study, we generated a preliminary model to characterize subgroups of ACC-survivors with high-risk symptom cluster profiles (Finnegan et al, 2009). Using data from over 7000 CCSS participants, we are now refining and testing this model in the Symptom Cluster Experience Profile study.…”
