In recent years, increased attention has been focused on antidepressant-associated sexual dysfunction, largely because of the widespread use of serotonin-specific reuptake inhibitors (SSRIs) and the recognition that such side effects can have a negative impact on treatment compliance. Data suggest that serotonergic antidepressants are associated with delayed ejaculation and anorgasmia, although these sexual problems are also linked to depression and to age. In this review, we discuss central mediators of normal orgasmic functioning and dysfunction, the relationship between depression and sexual dysfunction, possible mechanisms for SSRI-associated sexual dysfunction, and evolving treatment strategies.

Factors Influencing Glycemic Control in Type 2 Diabetes During Acute- and Maintenance-Phase Treatment of Major Depressive Disorder With Bupropion

OBJECTIVE -Depression management in both short-and longer-term treatment studies has been associated with improvement in glycemic control. We used bupropion hydrochloride (Wellbutrin XL) to determine whether this improvement could be attributed to changes in anthropometrics or diabetes self-care.RESEARCH DESIGN AND METHODS -Ninety-three patients with type 2 diabetes and major depressive disorder (MDD) received bupropion hydrochloride in a two-phase, openlabel treatment trial. Those who completed the acute phase (10 weeks; n ϭ 75) and whose depression remitted (n ϭ 63) continued bupropion at the remission dose and were followed in the maintenance phase (24 weeks) until attrition (n ϭ 8) or relapse of MDD (n ϭ 0). Self-report scales were used to measure depression symptom severity and diabetes self-care behaviors. Body composition and glycemic control were determined using dual-energy X-ray absorptiometry and serial determinations of A1C.RESULTS -BMI, total fat mass, and A1C decreased and composite diabetes self-care improved over the acute phase (Ϫ0.5 kg/m 2 , Ϫ0.7 kg, Ϫ0.5%, and ϩ0.4, respectively, P Ͻ 0.01 for each), effects that persisted through the maintenance phase for BMI, A1C, and self-care (P Յ 0.01 for each). Reductions in BMI (B ϭ 0.30, P ϭ 0.01) and depression severity (B ϭ 0.04, P ϭ 0.046) independently predicted lower A1C after acute-phase treatment, whereas only reduction in depression severity (B ϭ 0.08, P ϭ 0.001) predicted A1C over the maintenance interval.CONCLUSIONS -In the short term, improvement in glycemic control during bupropion treatment is predicted independently by improvements in mood and body composition. Longerterm improvements in glycemic control are predicted primarily by sustained improvement in mood via mechanisms independent of anthropometric and self-care modifications. Background: Antidepressant-induced sexual dysfunction (SD) is a common problem, associated with a significant risk of non-adherence. Selective Serotonin Reuptake Inhibitors (SSRIs) are associated with a substantial risk of SD. Only 10 % of patients show spontaneous improvement during follow up period. Objectives: This study aimed to compare two proposed medication (bupropion vs. amantadine) in alleviating SD in patients treated with SSRIs. Patients and Methods:In a randomized, single-blinded, clinical trial in Iran, 46 patients were recruited based on DSM-IV-TR criteria and semi-structured interview. Then, they were randomized into two treatment groups using table of random numbers. Eight patients were excluded and finally 38 patients completed the study which lasted for 4 weeks. Twenty patients were given bupropion, 18 patients were randomly assigned to another group, and given amantadine. Patients were assessed with the Arizona sexual experience scale (ASEX) at baseline and 4 weeks after the treatment. Results: A total of 38 patients completed the study (18 patients in amantadine vs. 20 patients in bupropion).The mean ASEX scores gradually declined in both study groups during the trial. The reduction of ASEX score in bupropion group was more than that of amantadine group that was statistically significant. So, the addition of bupropion at higher doses appears to be more effective approach in comparison with amantadine. Conclusions: These results provide empirical support for conducting a further study on comparing different add-on strategies for treating drug-induced SD.
