Susceptibility‐guided sequential strategy versus empirical therapy for Helicobacter pylori infection: study protocol for a randomised controlled trial

Abstract: Background New treatment strategies are required against infections caused by Helicobacter pylori, which grows increasingly resistant to antibiotics. Polymerase chain reaction-based methods for antibiotic susceptibility testing are available for detecting H. pylori-specific mutations that confer resistance to clarithromycin and levofloxacin. Several meta-analyses have compared eradication rates for susceptibility-guided versus empirical therapy for H. pylori treatment; however, all have signifi… Show more

“…Third, we did not test for susceptibility, but various studies suggest that the evidence is too limited to support the generalized use of susceptibility‐guided therapy for H . pylori treatment in routine clinical practice, either as a first‐line or rescue treatment 35,36 and larger and more individualized RCTs are needed to aid clinical decision‐making 37 . Fourth, we did not assess 24‐h intragastric pH values during the treatment and the CYP2C19 genotypes.…”
