Survival of Post-Transplant Lymphoproliferative Disorder after Kidney Transplantation in Patients under Rapamycin Treatment

Abstract: Background: One of the important causes of mortality and morbidity in kidney transplanted patients is Post Transplant Lymphoproliferative Disease (PTLD), which is due to immunosuppression therapy and viral activity. It seems that Rapamycin, with dual antineoplastic and immunosuppressive effects, may have a pivotal role in the treatment of PTLD patients and preserving transplanted kidneys. Methods and Materials: Twenty patients with PTLD were enrolled.  Immunosuppressive therapy was reduced or ceased, and… Show more

“…In addition, patients only on mTORi maintenance therapy have developed PTLD 113 . There are several relatively small retrospective reviews or case series, with no controlled studies, that suggest that replacing calcineurin inhibitors with an mTOR inhibitor for a variety of reasons including the development of PTLD or high EBV load may be an effective immunosuppressive therapy 114–118 . However, it should be noted that a prospective study of everolimus and reduced calcineurin inhibitors therapy in pediatric liver transplant patients was stopped prematurely due to high rates of PTLD, presumably due to over immunosuppression 119 .…”

“…Successful remission of refractory T-PTLD has been documented in only a limited number of studies, primarily case series and reports. Despite these efforts, the mortality rate associated with T-PTLD remains extremely high [ 5 ].…”
