Survival of Block and Fascial-Wrapped Diced Cartilage Grafts

Abstract: The amplitude of resorption of the diced cartilage wrapped in fascia is considerable compared with one-piece block grafts. It may be anticipated that the enthusiasm for this technique will decline once the long-term results of pertaining clinical studies are available.

“…However, most previous studies investigating cartilage graft survival have used this technique [1,2,8,9,15]. The results of our study show the superior viability and reduced resorption rates of the perichondrium-wrapped diced cartilage grafts as compared with the fascia-wrapped graft.…”

“…In a later study by Daniel and Calvert, high resorption rates were reported from Surgicel Ò -wrapped cartilage and the use of fascia was recommended [4]. Conversely, a comparison between block and fasciawrapped diced cartilages found that fascia wrapping caused low viability rates [15].…”

“…Due to the inflammatory response and fibrosis there is subsequent resorption of the diced cartilage. 7 Firat et al have reported better results with use of bare cartilage than wrapping it with fascia. 8 Excellent results have been reported with use of multifragmented cartilage graft wrapped in fascia in another series.…”

“…21 Absorption of diced cartilage fascia grafts was found to be clinically insignificant, obviating the need for overcorrection. 22 The potentially crucial role of the fascial sleeve for cartilage survival 5 has been questioned recently by 2 studies 23,24 comparing survival of solid block and diced cartilage wrapped in fascia in rabbits. The degree of resorption of the diced cartilage wrapped in fascia was found to be considerable compared with 1-piece block grafts, most likely because of histologic evidence of fibrosis and inflammatory cell infiltration.…”

“…32 It is now accepted that this variation may be the result of the degree of crushing, and cartilage survives well compared with other autologous tissues, even if transplanted without a sleeve. 15,23,24 This suggests replacing the fascial sleeve by a binder holding the cartilage in place. A prerequisite for the concept of bonding is cartilage survival and graft stability without the resorption seen with oxymethylcellulose sheeting and acrylate glue.…”
