Objective: To evaluate the success rate of Bollard miniplate anchorage for bone anchored maxillary protraction (BAMP). Materials and Methods: Twenty-five consecutive patients (mean age, 12.0 6 1.2 years; range, 8.7-14.8 years) with maxillary hypoplasia without congenital or acquired deformation were included in this study. A total of 100 Bollard modified miniplates were placed by the same surgeon. Ninety-nine miniplates were inserted under general anesthesia, and one was placed under local anesthesia because of initially soft bone conditions. Loading of the miniplates with 150 g elastics was initiated at 17.5 6 6.9 days (range, 11-38 days) after surgery. Mean follow-up was provided at 20.8 6 11.1 months (range, 6.5-46.2 months). Results: The overall success rate of miniplate anchorage in terms of stability was 97%. During orthodontic loading, five miniplates showed signs of mobility. After interruption of loading over 2 months, two miniplates became stable again. However, a total of three miniplates needed to be removed and were successfully replaced under local anesthesia after a mean healing period of 3 months. Conclusion: Skeletal anchorage by means of Bollard modified miniplates is effective for BAMP. Success depends on proper presurgical patient counseling, minimal invasive surgery, good postsurgical instructions, and orthodontic follow-up. (Angle Orthod. 2011;81:1010-1013

Objective: To investigate whether resonance frequency analysis (RFA) is suitable to measure orthodontic mini-implant stability. Implant size significantly affects the level of resonance frequency. Regarding the operating mode of RFA, it has to be proven whether the resonance frequency of mini-implants in bone fits the range of frequency emitted by the Osstell ISQ device. Material and Methods: For this purpose the SmartPegs in the Osstell ISQ device were modified to fit with the inner screw thread of orthodontic mini-implants, and 110 mini-implants were inserted into porcine pelvic bone. RFA was performed parallel and perpendicular to the run of superficial bone fibers. A suitability test, Periotest, was also performed in the same directions. Compacta thickness was measured using cone-beam computed tomography. Correlation tests and linear regression analysis were carried out between the three methods. Results: The RFA showed a mean Implant Stability Quotient value of 36.36 6 2.67, and the Periotest mean value was 22.10 6 1.17. The differences between the two directions of measurement were statistically significant (P . .001) for RFA and the Periotest. There was a high correlation between RFA and the Periotest (r 5 20.90) and between RFA and compacta thickness (r 5 0.71). The comparison between the Periotest and compacta thickness showed a correlation coefficient of r 5 20.64. Conclusion:The present results suggest that RFA is feasible as a measurement method for orthodontic mini-implant stability. As a consequence, it could be used for clinical evaluation of current stability and allow stability-related loading of mini-implants to reduce the failure rate. (Angle Orthod. 2013;83:230-238.)

The term "mini-screw" is one of a number used to describe small screws, surgically placed through alveolar cortical bone to be used as temporary anchorage devices in orthodontic treatment. They have become increasingly prevalent in the past decade as they confer a number of advantages when compared to traditional intra-oral and extra-oral anchorage reinforcement.There is marked heterogeneity in mini-screw design and placement techniques. This review was constructed with the aim of analysing the success rates of mini-screws with a view to defining a set of guidelines for their selection and application. The validity of this review is compromised somewhat by the use of only one database in collating any relevant articles; the paucity of search terms is also discouraging. However, upon comparison with other relevant papers, 1,2 it does appear that the studies included are representative of the available literature. A detailed analysis of each article would also have been desirable; however, information such as the design of each study was unreported. Given one of the main aims was to discern the success rates of mini-screws, a discussion of what is deemed a success would seem to be appropriate. Although not forthcoming, it is pertinent to realise that 'success' differs in the discussed articles. It is most frequently seen as gaining 'anchorage for required treatment time' and so mobile/displaced mini-screws can still be seen as a success provided they can still be used to reinforce anchorage.Accepting these issues, the quoted success rate of over 80% is comparable with that derived in other reviews on the subject. Of worthy mention is one such paper containing more contemporary data on an additional five studies yielding an increase of over 800 mini-screws in its analysis. 3 However, the overall success rate changes little.When comparing the time allowed for the mini-screw to heal before loading, along with the magnitude of the subsequently applied force, there was considerable variation between the studies and within study groups. Along with the poorer performance observed with mini-screws of less than 1.2mm diameter and 8mm in length, these areas generate further avenues for investigation.Given that this area of orthodontic research is still in its infancy, the data shown here can be seen as a starting point for the design of suitable prospective studies to help elucidate the most efficacious method for mini-screw placement.
