Survival after Lung Metastasectomy in Colorectal Cancer Patients with Previously Resected Liver Metastases

Abstract: Resection of lung metastases from CRC patients may result in prolonged survival, even after previous hepatic metastasectomy. Yet, prolonged disease-free survival remains the exception, and seems to occur only in patients with a single lung lesion.

“…Like others have reported [20,21], we could not demonstrate any impact of primary tumor location on overall survival. However, we are unaware of any published report showing a relationship between the site of the primary CRC and the pattern of recurrence.…”

“…We have excellent follow up post RFA treatment, which seems to be as good as other published results, with our longest follow up out to 11 years. In surgical series, completeness of resection and distribution and size of pulmonary deposits are prognosticators for survival 10,11,24,27,33,34 and median survival of 46 months can be achieved after resection. 33 RFA provides good local control for lung metastases.…”

“…3,6,17 Surgical metastasectomy to lung deposits can provide local control, improved disease free survival and is associated with a better 5-year survival when historically compared to chemotherapy alone. 10,11,24,27,33,34,38 Two registries in Denmark and Australia, analysing nearly 3500 patients demonstrated a superior survival for patients receiving systemic chemotherapy and undergoing resection of their primary tumour and pulmonary deposits as compared to patients not treated with Abbreviations: CEA, carcinoembryonic antigen; CT, computed tomography; PET, positron emission tomography. surgery.…”

“…The 5-year survival rate of patients treated with pulmonary metastasectomies was found to be 55%-67% [91,92] . The median disease-free survival (DFS) and OS for those who had both hepatic and pulmonary resection has been shown to be 13-19.8 mo and up to 87 mo, respectively [93,94] . Bevacizumab is the only anti-angiogenic agent that has been extensively studied in the setting of resectable (or potentially resectable) mCRC.…”
