Electronic health records (EHRs) are increasingly employed to maintain, store and share varied types of patient data. The data can also be utilized for various research purposes, such as clinical trials or epidemic control strategies. With the increasing cost and scarcity of healthcare services, healthcare organizations feel at ease in outsourcing these services to cloud-based EHRs. That serves as pay-as-you-go (PAYG) "e-health cloud" models to aid the healthcare organizations handling with existing and imminent demands yet restricting their costs. Technologies can host some risks; hence the privacy of information in these systems is of utmost importance. Regardless of its increased effectiveness and growing eagerness in its adoption, not much care is being employed to the privacy issues that might arise. Privacy preservation need to be reviewed about the changing privacy rules and legislations regarding sensitive personal data. Our work aims at answering three major questions: firstly, how privacy models and privacy techniques correlate with each other, secondly, how we can fix the privacy-utility-trade off by using different combinations of privacy models and privacy techniques and lastly, what are the most relevant privacy techniques that can be adapted to achieve privacy of EHR on cloud.

State-of-the-art progress in cloud computing encouraged the healthcare organizations to outsource the management of electronic health records to cloud service providers using hybrid cloud. A hybrid cloud is an infrastructure consisting of a private cloud (managed by the organization) and a public cloud (managed by the cloud service provider). The use of hybrid cloud enables electronic health records to be exchanged between medical institutions and supports multipurpose usage of electronic health records. Along with the benefits, cloud-based electronic health records also raise the problems of security and privacy specifically in terms of electronic health records access. A comprehensive and exploratory analysis of privacy-preserving solutions revealed that most current systems do not support fine-grained access control or consider additional factors such as privacy preservation and relationship semantics. In this article, we investigated the need of a privacy-aware fine-grained access control model for the hybrid cloud. We propose a privacy-aware relationship semantics-based XACML access control model that performs hybrid relationship and attribute-based access control using extensible access control markup language. The proposed approach supports fine-grained relation-based access control with state-of-the-art privacy mechanism named Anatomy for enhanced multipurpose electronic health records usage. The proposed (privacy-aware relationship semantics-based XACML access control model) model provides and maintains an efficient privacy versus utility trade-off. We formally verify the proposed model (privacy-aware relationship semantics-based XACML access control model) and implemented to check its effectiveness in terms of privacyaware electronic health records access and multipurpose utilization. Experimental results show that in the proposed (privacyaware relationship semantics-based XACML access control model) model, access policies based on relationships and electronic health records anonymization can perform well in terms of access policy response time and space storage.

The international healthcare systems interoperability is an unresolved technological area at the moment. This paper demonstrates the results of the software engineering research for simplex syntactic and semantic technical interoperability of hospital information systems, eHealth smart device technology and clinical telemedicine instruments through the recently developed Telemedicine Interoperability Hub. Several similar experiments exist. This research is unique in building a prototype interconnecting not only healthcare information systems with each other, but aiming to establish a general healthcare interoperability scheme including also the eHealth smart devices and telemedicine instruments. The aim of this research is to establish cloud-based data interchange capability with the newly developed information technology system interconnected with the emerging eHealth Internet of Things solutions and the classical hospital information system architecture. Notwithstanding the international information technology medical data exchange standards, like Health Level Seven, the adoption of an industry-wide open telemedicine syntactic and semantic interoperability standard is necessary.
