Patellar instability can significantly influence the locomotor function in children with Down's syndrome. The aim of this study was to evaluate the mid-term results of the operative treatment of patellar instability in children with Down's syndrome. The study included eight children (10 operated knees) with Down's syndrome and associated patellar instability. The children's age ranged from 6 to 11 years (the mean age was 7 years 9 months). The operative treatment involved Green's quadricepsplasty in six cases (eight knees) and Green's quadricepsplasty augmented with a modified Galeazzi procedure - semitendinosus tenodesis - in two cases. The mean follow-up period was 3 years and 3 months. We achieved a stabilization of the patellofemoral joint and a correction of the position of the patella in seven knees (five of these were treated with Green's procedure and in two cases Green quadricepsplasty was combined with the Galeazzi procedure). We did not observe any recurrence of patellar dislocation in this group during the follow-up period. We noted two failures, defined as a recurrence of dislocation, during the mean of 9 months postoperatively. Green's quadricepsplasty provides satisfactory results in younger children with Down's syndrome. In older children, we recommend the modified Galeazzi procedure.

Doğuştan patellofemoral patolojiler, hem anatomik olarak normal gelişimini tamamlamış, hem de displastik gelişim göstermiş dizlerde görülebilir. Patellanın doğuştan çıkığı, doğum esnasında var olan ve ekstansör mekanizmanın tam olarak işlev görmesine engel olan bir bozukluktur. Doğuştan patella çıkığının, sekonder kemik deformiteleri gelişmeden önce erken dönemde tedavi edilmesi büyük önem taşımaktadır. Patellofemoral eklemin nadir görülen doğuştan sorunlarından biri de, ekstansör mekanizma duplikasyonudur. Tedavisinde hipoplastik olan yapı kesilir ve geride kalan baskın ekstansör mekanizma, santralize edilerek işlevsel bir hale getirilir. Tırnak patella sendromu genellikle patello femoral tutulum ile kendini gösteren bir diğer genetik bozukluktur. Hastalık, tırnaklarda bozukluklar, radius başı hipoplazisi veya tamamen yokluğu, posteriyor iliyak çıkıntılar üstünde veya kenarında iliyak boynuzların gelişmesi ile karakterizedir. Down sendromuna bağlı patellar instabilite ve çok parçalı patella kırıkları da, diğer patellofemoral bozuklar arasında sayılabilir. Özellikle patellanın superiyor ve lateral kadranında görülen çoklu ossifikasyon merkezleri sık karşılaşılan bir durumdur.

Patellar instability is a frequent cause of knee pain in childhood and adolescence. Treatment is usually conservative especially after a single episode of dislocation, although in cases of recurrence the surgical option needs to be evaluated. The techniques proposed involve only soft tissues in order to preserve the skeletal maturation. This study evaluates the long-term results on 20 knees in 19 adolescents affected by patellar instability treated with the distal realignment procedure involving medial transfer of the lateral hemi-patellar tendon according to Goldthwait technique, preceded by arthroscopy to perform lateral release. The results were analysed with the Cox grading system and showed 11 excellent cases, 6 good cases, 2 fair cases and 1 poor case. Similar findings were obtained with the Bray score.
