ÖzetAmaç. Stres üriner inkontinans (SUI) olgularında, Transobturator bant (TOT) operasyonunun sonuçlarının ve komplikasyonlarının, klinik ve yaşam kalitesi analizi yapılarak değerlendirilmesi. Yöntem. SUI veya mikst üriner inkontinans (MUI) tanısı ile TOT yapılan 30 hasta çalışmaya alındı. Hastaların yaş ortalaması 46,5±7 idi. Preoperatif ürolojik ve jinekolojik anamnez alındı ve jinekolojik muayene, ultrasonografi, stres test, Q tip test yapıldı, inkontinans anketleri dolduruldu. TOT operasyonu monoflaman tape kullanılarak dıştan içe yöntem kullanılarak yapıldı. Hastaların postoperatif dönemde tüm klinik ve laboratuar bulguları değerlendirilerek preoperatif parametreler ile karşılaştırıldı. Operasyondan sonra hastaların ortalama takip süresi 16,7 aydı. Sonuçlar Wilcoxon signed rank testi ile istatistiksel anlamlılık açısından değerlendirildi. Bulgular. Sekiz hastada (%26,7) SUI, 22 hastada (%73,3) MUI mevcuttu. TOT ameliyatına ek olarak 6 (%30) hastaya kolporafi anterior (KA), 1 (%3,3) hastaya kolporafi posterior (KP), 12 (%40) hastaya KA ve KP 2 (%6,6) hastaya vaginal histerektomi, KA, KP uygulanmıştır. Bir hastada mesane perforasyonu, bir hastada postoperatif urge inkontinans görüldü. TOT operasyonu ile hastaların 26'sında (%86,7) başarılı olunurken, dördünde (%13,3) başarısız olunmuştur. Yaşam kalitesi değerlendirilmesinde IIQ-7 (Incontinence impact questionnaire) ve UDI-6 (Urinary distress inventory) skorlarında istatistiksel olarak anlamlı düzelme saptanmıştır (P<0,001 ve P=0,026). Sonuç. Bu çalışmada, SUI tedavisinde TOT uygulaması ile yüksek objektif iyileşme ve yaşam kalitesinde anlamlı düzelme sağlandığı saptanmıştır. Uygulama kolaylığı ve komplikasyon oranının düşük olması sebebiyle TOT operasyonunun diğer inkontinans cerrahi prosedürlerine iyi bir alternatif olduğu sonucuna varılmıştır. In stress urinary incontinence patients (SUI), the evalution of the results and complications of Transobturator tape (TOT) procedure by using clinical and life quality analysis. Method. 30 patients, who are diagnosed as SUI or MUI and operated with TOT procedure were included in the study. The mean patient age was 46.5±7. Aim: The aim of this study is to evaluate the surgical outcomes and complications of transobturator tape (TOT) procedure. Material and Methods:In this study, 175 patients who were undergone TOT procedure due to pure stress urinary incontinence (SUI) or mixt urinary incontinence (MUI) from 2011-2014 and examined at 1 st and 3 rd postoperative months with physical examination and stress tests were evaluated retrospectively. Results:All of the patients were multiparous and the median age was 50.0±11.3 years. Sixty-eight (38.8%) of the patients were postmenopausal and sixty-two (35%) of the patients who had TOT operation had extra surgery. One patient is complicated with bladder perforation. Only one patient had blood transfusion requirement due to TOT operation. Intraoperative bleeding more than 200 ml. is seen in 6 patients. Four patients had vaginal fornix perforation. Three patients had developed perineal pain. De-novo urge incontinence was seen in 8 patients in the postoperative period and was successfully treated with anticholinergic drugs. Mesh removals and reconstruction of vaginal mucosa were done for 6 patients who had vaginal erosions. The objective and subjective cure rates were found to be 87.4% and 82.8%, respectively. Conclusion:The use of mid-urethral synthetic slingis the first-choice of the surgical treatment of SUI nowadays. The main reasons for this are low complication rates due to surgical technique, the use of good quality materials, short operation time, and short hospital stay.Keywords: transobturator tape, outside-in, outcomes, complications. Sonuç:Mid-üretralsentetik slingkullanımı SÜİ cerrahi tedavisinde günümüzde ilk seçenek olarak tercih edilmektedir. Kaliteli malzeme kullanımı ve cerrahi teknik sayesinde komplikasyon oranının düşük olması, operasyon süresinin kısa oluşu ve hastanede kalış süresinin az olması bunda esas etkili olan faktörlerdir.

Background: To assess the safety and efficacy of Trans-obturator tape (TVT-O) for female stress urinary incontinence (SUI).Methods: A cohort of 35 cases of SUI and underwent TVT-O procedure over a period of 4 years. Pre-operative evaluation included pelvic examination, one hour pad test, urodynamic study, urinary diary and baseline haematological tests. Post-operatively all women had post-void residual volume and were followed up for 3 years.Results: The mean age of the patients was 47±9.5 years. TVT-O alone was done in 58.1% of cases. 41.9% of cases had TVT-O done as a concomitant procedure. The median duration of follow-up was 48 months. The overall objective and subjective cure rate was 92.3% with failure rate of 7.7%, 12.9% of women reported post-operative thigh pain and 6.5% patients had immediate post-operative urinary retention. 16.1% of the patients had post-operative voiding dysfunction. No major complications were encountered in these patients.Conclusions: TVT-O is a safe, easy and effective minimally invasive procedure for female SUI with minimal acceptable complications.
