BACKGROUNDIntravenous leiomyomatosis (IVL) is a rare and complicated disease, which requires surgery by a multidisciplinary team. However, the optimal surgical approach has not been determined.CASE SUMMARYHere we report three cases of IVL treated with different surgical approaches. All patients presented with circulation symptoms. Two patients had lower extremity edema and the other had cardiopalmus. The diagnosis of IVL was confirmed based on the imagining examinations and pathological findings. All patients underwent surgical treatment and were discharged without any complications.CONCLUSIONPreoperative examination is crucial for surgical planning and surgical approach is dependent on the patient’s condition and tumor involvement.

BackgroundUterine intravenous leiomyomatosis (IVL), a rare type of uterine leiomyoma, is defined by the intravascular proliferation of a histologically benign smooth muscle cell tumor. Pelvic arteriovenous fistula (AVF) is a rare vascular malformation that is most commonly congenital, post-traumatic, or iatrogenic. The link between leiomyomatosis and AVF has received little attention in the medical literature.ResultsWe provide a case series of seven patients, four of whom were from our center, who had IVL complicated by a pelvic AVF. The symptoms of right heart failure were noted as swelling in the abdomen and two legs as well as a significant amount of ascites. Coil embolization of AVFs may be beneficial in minimizing bleeding during IVL surgery. A review of all accessible literature published on IVLs from 2000 to 2020 was conducted, and data were retrieved from 78 papers totaling 262 cases. Complications and recurrence were associated with pelvic mass excision and intravascular remnant tumor, respectively.ConclusionIntravenous leiomyomatosis combined with AVF aggravates congestion symptoms of surrounding organs. It is worth noting the uncommon combination of AVF and IVL, stressing the importance of a thorough assessment and surgical approach in IVL treatment.

Intravenous leiomyomatosis (IVL) is a rare and benign smooth muscle tumor that arises from intrauterine venules or the myometrium. We herein describe a 49-year-old woman with a history of myomectomy who developed abdominal pain. An intravascular mass with extension to the right atrium was detected in the inferior vena cava. The mass was surgically resected in a single stage under cardiopulmonary bypass. IVL features were indicated by subsequent histopathology. Postoperatively, the patient was diagnosed with massive pericardial effusion and treated with a pericardial window. At 3 months’ outpatient clinical follow-up, she was asymptomatic. This case indicates that the diagnosis of IVL with extension to the heart should be kept in mind in patients presenting with abdominal pain.
