Surgical Therapy in Intractable Benign Paroxysmal Positional Vertigo

Abstract: Singular neurectomy (posterior ampullary nerve transsection) and posterior semicircular canal occlusion are the 2 specific techniques used in intractable BPPV surgery. The numbers of operated cases are 342 and 97, respectively. These small numbers indicate that the procedures are difficult and risk compromising hearing and that a very small population of patients require surgical treatment of BPPV. The operated cases have been decreasing since the early 1990s because of improved management in BPPV. This articl… Show more

“…Surgical treatments may be considered in these rare occasions of so-called intractable BPPV 84-87. Transection of the posterior ampullary nerve innervating the posterior canal (singular neurectomy) or posterior semicircular canal occlusion (canal plugging) have been performed for intractable PC-BPPV 84…”

“…As CRM become more specific, the need for surgery is decreasing; however, surgery is an extremely effective measure to relieve BPPV. The patient in our report has been free of symptoms for over 1 year since the surgery, demonstrating a successful procedure for only the second reported case of surgical intervention for SSC BBPV.…”

“…Symptoms are often self‐limited and resolve spontaneously. Canalith repositioning maneuvers (CRM) have proven to be extremely effective for PSC BPPV, with a success rate of up to 98% . Despite an anatomical orientation that favors resolution of canalithiasis, SSC BPPV has proven to be refractory to CRM in some cases .…”

“…3,4 Ampullary nerve section is reserved for more serious cases, since it is both difficult and risky. 5 Recent advances in medical laser techniques have made laser occlusion of the posterior semicircular canal more attractive. Antonelli et al reported that this surgical procedure blocked the membranous semicircular canal more effectively, leading to faster post-operative rehabilitation, and had significantly less vestibular complications, compared with mechanical occlusion.…”
