Surgical techniques for aortic valve xenotransplantation

Abstract: Background Heart valve replacement in neonates and infants is one of the remaining unsolved problems in cardiac surgery because conventional valve prostheses do not grow with the children. Similarly, heart valve replacement in children and young adults with contraindications to anticoagulation remains an unsolved problem because mechanical valves are thrombogenic and bioprosthetic valves are prone to early degeneration. Therefore, there is an urgent clinical need for growing heart valve replace… Show more

“…15 If xenotransplantation of piglet organs, tissues, and cells could be implemented, organs would be available almost immediately and may be feasible in patients that currently may not qualify for organ transplantation. 32 Genetically modified porcine to human cardiac xenotransplantation has recently been attempted, but the xenograft failed from diastolic ventricular failure. 33 The longest reported survival time of pig heart transplant into nonhuman primates is around 500 days.…”

“…Xenotransplantation stands to provide a solution to this issue, potentially producing donor grafts in unlimited supply 15 . If xenotransplantation of piglet organs, tissues, and cells could be implemented, organs would be available almost immediately and may be feasible in patients that currently may not qualify for organ transplantation 32 . Genetically modified porcine to human cardiac xenotransplantation has recently been attempted, but the xenograft failed from diastolic ventricular failure 33 .…”

“…In the future, this may be addressed using partial heart xenotransplantation. 38,39 Second, the cost for partial heart transplant is likely similar to the cost of OHTs. Third, due to the technical nature of this procedure, it is best performed by experienced surgeons in a medium- to high-volume cardiac transplant center to ensure the best outcomes for patients.…”
