Résumé -Introduction : La gestion d'une alvéole post-avulsionnelle représente un défi quotidien. En effet, si elle n'est pas contrôlée, la résorption osseuse verticale et horizontale associée peut rendre difficile le positionnement tridimensionnel idéal de l'implant et l'intégration esthétique de la future prothèse. Le but de cette publication technique est d'illustrer les différentes étapes d'une stratégie chirurgicale de préservation alvéolaire et de mise en place de l'implant en deux temps, de manière à optimiser la restauration prothétique. Objet : Dans un premier temps, l'avulsion est réalisée de la manière la plus atraumatique possible et l'alvéole soigneusement curetée. Le site opératoire est ensuite préservé à l'aide d'une membrane collagène et de sutures résorbables, sans rechercher de fermeture hermétique. L'obtention d'une cicatrisation gingivale de deuxième intention signe le succès de la technique. La temporisation est assurée par une prothèse provisoire non compressive. Après 16 semaines et après contrôles clinique et radiologique, l'implant est placé dans le site osseux cicatriciel de manière idéale dans les trois plans de l'espace. Une chirurgie muco-gingivale assure une esthétique optimale. La mise en fonction implantaire se fait 12 semaines plus tard sur un site sain, de la manière la plus esthétique possible. Conclusion : Une préservation alvéolaire optimale grâce à une technique chirurgicale adaptée et une gestion des tissus péri-implantaires rigoureuse permet de réaliser de manière prédictive une restauration prothétique qui donnera entière satisfaction au patient.Abstract -Socket preservation using a xenograft material and a collagen membrane prior to an implant placement: technical note. Introduction: Management of a post-extraction socket is challenging. Indeed, uncontrolled, the vertical and horizontal bone resorption could disrupt the ideal tridimensional placement of an implant. The main goal of this article is to illustrate, step by step, an appropriate surgical technique to preserve the dental socket and to optimise the placement of the implant and the consecutive aesthetic prosthetic restoration. Object: Firstly , an atraumatic dental extraction is carried out and the socket carefully curetted. Then, the surgical site is preserved with a collagen membrane and resorbable sutures, not requiring a hermetic closure. Second-line gingival healing indicates the success of the procedure. During the healing time a non-compressive temporary prosthesis is used . After 16 weeks and after clinical and radiological controls, the implant is placed in an osseous healing site in an ideal way in the 3 planes of the space. Mucogingival "Mucogingival" surgery ensures an optimal aesthetic incorporation. Twelve weeks later, implant loading is carried out in a safe site, in the most aesthetic way possible. This article reviews the normal and pathological healing processes that take place after tooth extraction in orthodontic cases, and their associated complications within the mucosa or alveolar socket, such as gingival clefts or bone defects. The general and local factors that are involved in such deficient healing cases are detailed, in parallel to surgical procedure to enhance ridge preservation or to 'regenerate' tissues. The relationships between the orthodontist and periodontist are underlined, because both praticioners assess patient's risk factors and follow him during this treatment stage.

PurposeResorption of the alveolar bone is an unavoidable consequence of tooth extraction when appropriate alveolar ridge preservation (ARP) measures are not taken. The objective of this trial was to test the hypothesis that dimensional changes in the alveolar bone after tooth extraction would be reduced by inserting an equine collagen membrane and a collagen cone to fill and seal the alveolus (as ARP), in comparison to extraction with untreated alveoli.MethodsIn this randomized clinical trial, 31 patients were directly treated with the collagen material after extraction of a tooth from the maxilla (the ARP group). Twenty-nine patients served as the control group. After extraction, no further treatment (i.e., no socket preservation measures) was performed in the control group. Changes in the alveolar process immediately after extraction and after an 8 (±1)-week healing period were evaluated 3-dimensionally. Blinded analyses were performed after superimposing the data from the digitalized impressions and surfaces generated by cone-beam computed tomography.ResultsBoth the ARP and control groups showed a reduction of bone in the alveolar area after tooth extraction. However, significantly less bone resorption was detected in the clinically relevant buccal region in the ARP group. The median bone reduction was 1.18 mm in the ARP group and 5.06 mm in the control group (P=0.03).ConclusionsThe proposed hypothesis that inserting a combination material comprising a collagen cone and membrane would lead to a difference in alveolar bone preservation can be accepted for the clinically relevant buccal distance. In this area, implantation of the collagen material led to significantly less alveolar bone resorption. German Clinical Trials Register at www.drks.de, DRKS00004769.
