Surgical technique for reconstruction of the nasal septum: The pericranial flap

Abstract: Background We describe a new technique for the surgical reconstruction of large‐sized anterior septal perforations based on the pericranial flap. Methods The technique requires a standard open rhinoplasty combined with a pericranial flap harvested after a bicoronal approach and tunnelled to the nasal cavity. We present the case of a man with complete destruction of the nasal septum as a result of chronic cocaine abuse. Results Surgery resulted in a permanent and complete closure of the perforation. Conclusions… Show more

“…It has been used for the reconstruction of nasal septal defects, coverage of cochlear implants, reconstruction of external auditory canal following mastoidectomy, periorbital soft tissue reconstruction, coverage of soft tissue defects with bone exposure such as in full thickness avulsive scalp defects due to trauma, and reconstruction of oropharyngeal defects due to cancer resection 3,[7][8][9][11][12][13]23 . The pericranial flap has also been extensively used in craniofacial reconstruction to provide soft tissue bulk in congenital or acquired facial deformities and in skull base reconstruction 1,2,6,17,18 .…”

“…This technique is based on two mucoperichondrial flaps (one from each side of the septum) that is used to close the perforation and is a simple procedure. Extranasal flaps such as the labial-buccal flap [17], pericranial flap [18] or radial forearm free flap [19] is uncommonly done for large septal perforation. These methods usually have significant donor site morbidity.…”

“…As in other types of reconstructive surgery [16], a rotational flap offers the benefit of preserving the vascular supply. Plastic and reconstructive surgeons have successfully used vascularized pericranial flaps for reconstruction of congenital cranial malformations [17][18], repair of chronic scalp ulcers [19], dura mater reconstruction after decompressive craniectomy complicated by infection [20], orbital [21][22] and nasal septum [23] reconstruction, repair of supratarsal sulcus depression [24], cranial base reconstruction after tumor resection [25], and frontal sinus fracture repair [26]. In this technical note, we utilize the principles of plastic and reconstructive surgery in the implementation of a rotational pericranial flap for closure of a complex posterior fossa wound.…”
