Surgical Target Selection in Cerebral Glioma Surgery: Linking Methionine (MET) PET Image Fusion and Neuronavigation

Abstract: MET PET image fusion may facilitate the targeting of anaplastic foci in homogeneous MRI non-enhancing gliomas for biopsy, may identify oligodendroglial histology preoperatively as well as characterize biologically active tumor volumes within MRI T(1)/FLAIR tumor areas of candidate patients for resection.

“…Importantly, areas with highest MET and FDG uptake were concordant in those gliomas, which showed an increased glucose metabolism. The value of MET-PET for targeting the biopsy is supported by the results of Roessler et al [33]: an increased MET uptake was detected in 26 of 27 gliomas, and allowed detection of anaplasia in five gliomas showing no Gd enhancement in MRI. The uptake in gliomas and the image contrast of FET appear to have similarities to those of MET [25].…”

“…In this respect, it is important to note that anaplastic gliomas are usually non-contrast-enhanced on MRI, and neoplastic cells almost always are found beyond areas of abnormal signal intensity [5]. PET with radiolabelled amino acid tracers has proved particularly useful in this challenging surgical situation [12,22]. Nevertheless, all patients with anaplastic gliomas in our series showed T1-Gd enhancement and therefore this sequence was used for surgery planning.…”

“…Some groups have used MET PET for the management and surgery planning of brain tumours [21][22][23]. Pirotte et al have analysed the specific contribution of PET and MRI to the definition of the target volume and propose a two-step classification of the relative location of the projection of PET and MRI volumes after visual analysis.…”

“…Roessler et al [36] looked at 27 patients with PET-targeted biopsies and found anaplastic foci could be recognised to aid biopsy targeting. Sadeghi et al [21] demonstrated in 14 patients that methionine (MET) PET provides an index of focal malignant activity.…”
