Aim: The aim of this study was to determine the clinicopathological characteristics, treatment details and outcome of patients with brain metastasis from epithelial ovarian carcinoma (EOC). Methods: This study included 21 patients diagnosed with brain metastasis from EOC between 1999 and 2009. Results: Median age was 61 years (range 38-77). The median time elapsed from EOC diagnosis to brain metastasis detection was 32 months. Single brain metastases were found in 10 (48%) cases, and there was extra-cranial disease in 11 (52%) cases. During the mean 86 months of follow-up, 18 of the patients (86%) died of the disease and 3 (14%) were alive with disease. The median survival time after the initial diagnosis of brain metastasis was 9 months. The median overall survival (OS) from initial diagnosis of EOC was 50 months. In univariate analysis, prolonged time from initial diagnosis to central nervous system metastasis (more than 32 months) (p = 0.001), treatment with radiotherapy (p < 0.001), optimal cytoreductive operation (p = 0.02) were all positively correlated with OS. Conclusion: The prognosis of patients with brain metastasis from EOC is still poor. The significant predictors of survival in our series were whole brain radiotherapy, prolonged elapsed time from initial diagnosis to brain metastasis and optimal cytoreductive surgery.

Ключевые слова: метастазы в головном мозге, внутричерепные новообразования, рак яичников.The review gives the data available in the literature on the problem in treating patients with intracranial metastases from ovarian cancer (OC). The incidence of secondary brain involvement in OC has increased 6.5-fold in the last 20 years. The optimal treatment policy for intracerebral metastases from OC remains a complicated problem. If the foci are more than 3 cm, surgical treatment followed by chemotherapy and radiotherapy is recommended. If the foci are smaller, brain stereotactic radiosurgery followed by chemotherapy is the method of choice. The authors' investigation was conducted in 8 patients with brain metastases from OC, who had undergone microsurgical removal of a metastatic brain tumor, followed by multimodality treatment. After multimodality treatment, median overall survival was 15.0 months and median relapse-free survival was 11.5 months. Metastatic brain involvement is traditionally considered as a final manifestation of the disease and characterized by an extremely pessimistic prognosis. Our experience demonstrates that combination treatment involving surgery results in prolonged survival in patients who have a satisfactory quality of life. Modern management of rare brain metastases in adults

Brain metastases (BM) represent the main cause of intracranial neoplasms in adults, while being relatively less common in children. Today, better treatment options of the primary malignancy lead to higher remission rates as well as prolonged stable clinical conditions. This may in part explain the increased incidence of BM. Morbidity and mortality rates in patients with malignancies deteriorate significantly in cases of metastatic involvement of the central nervous system. Nowadays, especially modern management using surgical, medical, and radiotherapeutic options for treatment of BM tends to improve survival rates and enhance quality of life. Nonetheless, almost all treatment options are considered as palliative. In this review, we outline current knowledge of the incidence, diagnostic facilities, and therapeutic management of rare BM, with consideration of the basic aspects of the primary malignancy.
