Surgical reconstruction of iliofemoral veins and the inferior vena cava for nonmalignant occlusive disease

Abstract: Venous reconstructions for iliofemoral or IVC obstruction offer 3-year patency rates of 62%. The Palma procedure with autologous saphenous vein had the best long-term patency, whereas long-term success with ePTFE was moderate. The use of an arteriovenous fistula to improve graft patency remains controversial.

“…A temporary arteriovenous fistula is often placed to improve flow and to aid patency. While clinical improvement is often achieved, long-term patency is quite poor with a secondary patency rate of 83 % at 48 months for autologous vein and 0 % in PTFE conduits [12]. Orthotopic iliac or iliocaval reconstruction can be performed for either unilateral venous occlusion when autologous conduit is not available or bilateral iliocaval, or inferior vena cava occlusion (IVC).…”

“…7 Surgical interventions including thrombectomy, bypass, reconstruction/replacement and interruption/ligation are used infrequently owing to inherent morbidity and mortality. [84][85][86] Thrombectomy may be indicated for acute or septic IVC thrombosis and PCD but these procedures are less commonly performed because of high rates of re-thrombosis. 7 Such procedures are typically completed in conjunction with a distal arteriovenous (AV) fistula to maintain high flow.…”

“…Jost et al [218] reported that venous reconstructions for iliofemoral or IVC obstruction offered 3-year patency rates of 62%. Puggioni et al [219] have performed surgical reconstitution of postthrombotic deep veins with endophlebectomy of 23 deep venous segments in 13 patients with advanced PTS.…”
