Abstract: Objective: In this study, we investigate our institutional experience of patients who underwent endoscopic endonasal transsphenoidal approach for treatment of large and giant pituitary adenomas emphasizing the surgical results and approach-related complications. Method: The authors reviewed 28 consecutive patients who underwent surgery between March, 2010 and March, 2014. Results: The mean preoperative tumor diameter was 4.6 cm. Gross-total resection was achieved in 14.3%, near-total in 10.7%, subtotal in 39.3… Show more

“…Patients presented with visual acuity complaints in 14–84% of cases (Supplementary Table 3) [ 5 , 36 ]. Ten studies evaluated postoperative visual acuity in their case series [ 5 , 6 , 9 , 20 , 22 , 25 , 30 , 36 , 37 , 46 ]. Pooled prevalence of overall improvement was 67.5% (95% CI: 59.1–75.0%, I 2 : 86.0%, n = 163/219).…”

“…First, they should make sure that the middle turbinate is retained; the lateral nasal wall should be adopted for a direct surgical approach when the patient has deviated nasal septum. [16] Second, the surgeons should pay attention to the conservation of the nasal mucosa. Third, the postoperative nasal packing should be performed precisely and gently; the middle turbinate should be fixed at the normal position, and the packing should be removed within 48 hours.…”

“…In addition, patients who already lost vision from these tumors can even achieve recovery following surgery. In literature reviews visual improvement after endoscopic endonasal Transphenoidal approach for large and giant pituitary adenomas excision ranges in between 32.1%-97.7% [13][14][15][16][17][18][19][20] respectively showing better results of endoscopic endonasal Transphenoidal approach for management of pituitary tumors in terms of visual outcome improvement.…”
