Surgical Management of Isolated Bilateral Adrenal Metastases from Colon Carcinoma Causing Adrenal Insufficiency

Abstract: Adrenal insufficiency due to histologically proven bilateral metastatic adrenal involvement from colon cancer has been clinically reported only twice. Surgical treatment of bilateral adrenal metastases is seldom performed, since other metastatic localizations are usually concomitant. To the best of our knowledge, we report the first case of relatively long-term disease-free survival after bilateral adrenalectomy in a patient with adrenal insufficiency due to bilateral metastases from colon carcinoma.

“…Laparoscopic adrenalectomy has become the 'gold standard' of treatment for benign adrenal masses (24). However, laparoscopy remains a controversial approach for known malignant diseases (3,23,25). For example, a previous study claims that adrenalectomy has not proven beneficial in the treatment of patients with adrenal metastases following resection of colorectal hepatic metastases (26).…”

“…It is rare for a patient to present with adrenal insufficiency secondary to bilateral adrenal gland metastasis from a malignant colorectal tumor [3], [4], [5], [6], [7]. The survival rate of patients with cancer is increasing, and improvements in imaging techniques have resulted in increased antemortem recognition of adrenal gland metastases.…”

“…It develops when >90% of adrenal tissue is destroyed [2]. It is rare for a patient to present with Addison’s disease secondary to bilateral adrenal gland metastases from a malignant colorectal tumor; only five cases have been published in the English literature [3], [4], [5], [6], [7]. Herein, we describe a sixth case of adrenal insufficiency in a patient with bilateral adrenal gland metastasis from rectal cancer, based on Surgical Case Report (SCARE) Guidelines [8].…”

“…16 Ultimately, the ensuing insufficiency, and that after bilateral adrenalectomy, can be controlled with hormone replacement therapy. 16,17 In conclusion, the management of adrenal metastasis from gastric cancer is controversial, and the rarity of such cases makes a randomized prospective trial comparing surgery with nonsurgical management unlikely. Although our patient's initial disease-free interval was less than 6 months and the adrenal metastasis was not solitary, the subsequent disease-free interval of 41 months and the current disease-free interval of longer than 3 years may justify the surgical resection of some metastatic lesions.…”
