Surgical Implantation of Artificial Urinary Device and Penile Prosthesis Through Trans-scrotal Incision for Postprostatectomy Urinary Incontinence and Erectile Dysfunction: Synchronous or Delayed Procedure?

Abstract: ObjectiveTo compare simultaneous and 2-stage surgical implantation of an artificial urinary sphincter (AUS) and penile prosthesis (PP) for patients with severe urinary incontinence and erectile dysfunction after prostatectomy. MethodsThe data from 23 patients were reviewed, 15 (group 1) underwent simultaneous implantation and 8 (group 2) underwent 2-stage surgery. “…Despite the growing evidence supporting dual implantation to manage both ED and SUI following RP, there are financial constraints placed upon reimbursement of two implants placed concurrently in the same patient by 3rd party payers. Many insurances and Medicare do not reimburse the whole cost of the second implant to the hospital and 50% the surgeon charge [11][12][13]. Because of the financial problems with doing a dual IPP+AUS implant, this study aims to assess the 30-day postoperative morbidity in patients undergoing combined placement of PP+AUS vs. PP+MS.…”

“…Rolle et al compared the simultaneous and the two-stage surgical implantation procedures, with data available for 15 and eight patients, respectively. 9 Results in terms of QoL were similar if devices were placed simultaneously or in a two-stage procedure. There were no differences in patient satisfaction or functional outcome between the two groups.…”

“…However, this was not recommended as long-term leakage due to muscle atrophy has been described. 9 Once cuff size was determined, air was evacuated from the cuff before final placement.…”
