The present study has demonstrated a new approach for adequately exposing the distal humeral articular surface during surgery for an intercondylar humerus fracture. With this modified approach, osteotomy of the olecranon and the separation or transection of the main part of the triceps brachial tendon from the olecranon are not necessarily required. Therefore, we suggest that this novel approach could be applied as the primary surgical approach in intercondylar humerus fracture surgeries if the surgeons are familiar with the regional features of distal tendon of the triceps brachii and anconeus.

ÖZETHumerusun ekleme yakın alt uç kırıklarının cerrahi tedavisinde olekranon osteotomisi mutlaka gerekli midir?Amaç: Humerus ekleme yakın alt uç kırığı nedeniyle açık redüksiyon ve paralel plak tekniği ile internal tespit uygulanan olgularda olekranon osteotomisinin gerekliliği ve klinik sonuçlar üzerine etkilerinin incelenmesi amaçlandı. Gereç ve Yöntem: Çalışmamıza dahil edilen 43 olgu kendi içlerinde; operasyonlar esnasında olekranon osteotomisi uygulanan (Grup 1) ve paratrisipital girişim uygulananlar (Grup 2) olarak 2 gruba ayrıldı. Olgular yatış, takip ve cerrahi süre, eklem hareket açıklıkları, komplikasyon gelişip gelişmediği ve fonksiyonel açıdan Mayo dirsek performans skorları ile değerlendirildiler. Bulgular: Olguların yaş ortalaması 44. 35±11.61 (19-62) yıldır. Gruplar arasında yaş, cinsiyet, taraf dağılımı, yatış süresi, takip süresi ve eklem hareket açıklıkları açısından istatistiksel olarak anlamlı bir farklılık bulunmamaktadır (p>0.05). Operasyon süreleri Grup 1 de ortalama 133.04±18.14 dakika, Grup 2 de 171±15.1 dakika olarak bulunmuş olup Grup 1'in operasyon süresi ortalaması, Grup 2'den istatistiksel olarak anlamlı düzeyde kısa bulunmuştur (p:0.001; p<0.05). Her iki grupta da cerrahi esnasında ve sonrasında her hangi bir komplikasyona rastlanmazken, radyolojik ve klinik olarak da kaynama ile ilgili bir sorunla karşılaşılmadı. Mayo dirsek performans skoruna göre Grup 1'de ortalama skor 85.65±4.34 ve Grup 2'de ortalama skor 90.25±4.13 olarak bulunmuştur. Bu sonuca göre Grup 1'de Mayo skoru ortalaması Grup 2'den istatistiksel olarak anlamlı düzeyde düşük bulunmuştur (p:0.001; p<0.05). Sonuç: Paratrisipital girişim yapılanlarda olumsuz olarak sadece cerrahi sure uzarken, avantaj olarak ise daha iyi hareket eklem açıklığı ve fonksiyonel sonuç elde edilmiştir. Sonuç olarak humerusun ekleme yakın alt uç kırığı nedeniyle açık redüksiyon ve paralel plak tekniğiyle internal tespit uygulanan olgularda olekranon osteotomisi eklemin bütünlüğünü bozmamak amacı ile zorunlu olmadıkça yapılmamalıdır. Purpose: To evaluate functions of the elbow joint according to surgical approach, time to exercise, and type of fracture after surgical treatment for the intra-articular comminuted fracture of the distal humerus. Materials and Methods: 27 patients with the intra-articular comminuted fractures of the distal humerus underwent surgery from March, 2000 to January, 2007. We investigated the surgical approach, time for union, time to exercise and age. We also evaluated postoperative functions of the elbow joint according to the flexion contracture, the range of motion and the Mayo elbow performance score. Results: The average follow-up period was 37 months and the average time for union was 14 weeks. The average range of flexion was 115 degrees, the average flexion contracture was 10 degrees, and the Mayo elbow performance score with average value of 85 point showed good clinical results. There were no statistically significant differences in functions of the elbow joint according to the operative method and age. However, patients with early postoperative exercise within 6 days showed statistically better outcomes than patients with postoperative exercise after 7 days. Type C1, 2 fractures showed statistically better results than the type C3 fracture. Conclusion: Stable fixation and early exercise are required to prevent postoperative complications and restore functions of the elbow joint with an intra-articular comminuted fracture of the distal humerus.
