Efficacy of Conventional Laser Irradiation Versus a New Method for Gingival Depigmentation (Sieve Method): A Clinical Trial

Introduction: Diode laser irradiation has recently shown promising results for treatment of gingival pigmentation. This study sought to compare the efficacy of 2 diode laser irradiation protocols for treatment of gingival pigmentations, namely the conventional method and the sieve method. Methods: In this split-mouth clinical trial, 15 patients with gingival pigmentation were selected and their pigmentation intensity was determined using Dummett's oral pigmentation index (DOPI) in different dental regions. Diode laser (980 nm wavelength and 2 W power) was irradiated through a stipple pattern (sieve method) and conventionally in the other side of the mouth. Level of pain and satisfaction with the outcome (both patient and periodontist) were measured using a 0-10 visual analog scale (VAS) for both methods. Patients were followed up at 2 weeks, one month and 3 months. Pigmentation levels were compared using repeated measures of analysis of variance (ANOVA). The difference in level of pain and satisfaction between the 2 groups was analyzed by sample t test and general estimate equation model. Results: No significant differences were found regarding the reduction of pigmentation scores and pain and scores between the 2 groups. The difference in satisfaction with the results at the three time points was significant in both conventional and sieve methods in patients (P = 0.001) and periodontists (P = 0.015). Conclusion: Diode laser irradiation in both methods successfully eliminated gingival pigmentations. The sieve method was comparable to conventional technique, offering no additional advantage.

ABSTRAKFrenulum labialis yang abnormal dapat berpengaruh terhadap kesehatan gingiva dan menimbulkan penyakit periodontal dengan cara menarik margin gingiva sehingga menimbulkan resesi gingiva. Abnormalitas dari frenulum ini juga menyebabkan diastema dari gigi insisivus sentral, iritasi pada jaringan periodontal, menghalangi proses pembersihan gigi, menghalangi pergerakan alat ortodonsi, mengganggu pemakaian protesa gigi serta berpengaruh pada estetik. Selain frenulum yang abnormal, masalah pada gingiva yang dapat berpengaruh juga pada estetik adalah pigmentasi gingiva. Pigmentasi pada gingiva merupakan hasil dari granul melanin yang diproduksi oleh melanoblas. Hiperpigmentasi melanin pada gingiva biasanya bukan masalah medis tetapi keluhan pasien yang menginginkan terapi perbaikan estetik. Untuk melaporkan penatalaksanaan frenektomi labialis superior dan depigmentasi pada kasus sentral diastema dan pigmentasi gingiva. Anak perempuan 11 tahun diastema sentral insisivus maksila disertai dengan hiperpigmentasi gingiva regio anterior maksila. Diastema sentral insisivus maksilanya disebabkan oleh perlekatan frenulum labialis superior yang tinggi. Perawatan untuk perlekatan frenulum labialis superior yang tinggi dilakukan frenektomi dan perawatan depigmentasi dilakukan dengan teknik scraping menggunakan skalpel. Perawatan frenektomi dan depigmentasi menunjukkan hasil perbaikan perlekatan frenulum dan menghilangkan hiperpigmentasi gingiva. MKGK. ABSTRACT: Management of Frenectomy and Gingival Depigmentation at Regio PENDAHULUANFrenulum merupakan lipatan kecil dari membran mukosa yang mengikat bibir atau pipi ke prosessus alveolaris dan berfungsi membatasi pergerakan pipi atau bibir. 1 Frenulum labialis superior adalah sisa dari struktur embrio yang menghubungkan tuberkula bibir atas ke papila palatina. Frenulum labial pada masa bayi normalnya mempunyai daerah perlekatan yang rendah di dekat puncak prosesus alveolaris atas di garis tengah. Pada periode gigi susu, frenulum labialis superior sering terlihat melekat pada prossesus alveolaris di antara gigi-gigi insisivus sentral atas. Bersamaan dengan pertumbuhan dentoalveolar yang normal, prossesus alveolaris atas akan tumbuh ke bawah dan daerah perlekatan frenulum labialis superior akan semakin rendah pada maksila. 2 Letak frenulum yang normal terhadap jaringan periodontal adalah melekat pada gingiva cekat sehingga pada waktu berfungsi tidak menimbulkan tarikan yang berlebih. 3

Excessive gingival pigmentation is a major aesthetic concern in modern society, though it is not a medical problem they consider it as a negative attribute. Patients with gingival hyperpigmentation usually complain and request cosmetic therapy, particularly if the pigmentation is visible during speaking and smiling. Various depigmentation methods, including burr abrasion, cryosurgery, electro-surgery, split thickness flap excision and surgical scraping techniques have been used with varying degrees of success. Recently, lasers have been used to ablate cells containing and producing the melanin pigment. The present study was undertaken to compare the clinical effectiveness and patient comfort of surgical scrapping and diode laser technique used for gingival depigmentation for a follow up period of 6 months.20 subjects participated in this split mouth study. The clinical evaluation parameters included Extent and Intensity of gingival hyperpigmentation, post-operative gingival bleeding and pain. On follow up examination at 6 th month there was no statistical difference in repigmentation extent and intensity between diode laser and surgical scraping techniques. The mean pain scores for treated sites with diode laser were significantly lower than surgical scrapping technique at 24 hours (t-value=2.430, p-value=0.02). The postoperative gingival bleeding at end of procedure was significantly lower with diode laser than surgical scrapping technique (p-value=<0.0001). There was no statistical difference in postoperative re-pigmentation and clinical efficacy among the subjects between surgical scraping and diode laser technique at 6 th month follow up. Diode laser technique provides better haemostasis and good visibility at the surgical site. The post-operative patient comfort is better at the surgical sites treated with diode laser than surgical scrapping method. Hence, both the techniques are used for depigmentation procedures depending on the severity and gingival biotype and patient acceptance.
