Surgery versus radical endotherapies for early cancer and high grade dysplasia in Barrett's oesophagus

Abstract: This Cochrane review has indicated that there are no randomised control trials to compare management options in this vital area, therefore trials should be undertaken as a matter of urgency. The problems with such randomised methods are standardising surgery and endotherapies in all sites; standardising histopathology in all centres; assessing which patients are fit or unfit for surgery; and making sure there are relevant outcomes for the study i.e. no progression of high grade dysplasia or long term survival … Show more

“…Tumour recurrence after resection of early cancer is rare (1%). There are no reports of long-term tumour recurrence in patients who have had complete surgical resection of HGD alone, and all-cause mortality is equivalent to non-surgical treatments211 290 291 (Evidence grade III).…”

“…New, less invasive radiological and treatment strategies are required, but these are already being introduced, such as laparoscopic surgery for colorectal carcinoma [37] and endomucosal resection of oesophageal cancers [38]. Some of the patients undergoing screening are likely to have anxieties related to the outcomes of their yearly tests, but these are arguably balanced by the knowledge that many cancers caught early are in most cases unlikely to be fatal.…”

“…A Cochrane review initially conducted in 2008 and subsequently updated in 2012 found 13 published comparative studies, although there have been no published RCTs. 108,109 Two papers from the Cochrane review were not included as they dealt only with EAC and not HGD. 110,111 Patients treated surgically in these trials have generally failed endoscopic treatment due to recurrent or extensive disease and, therefore, may have more aggressive or treatment-resistant disease as compared with those who had successful endoscopic therapy.…”
