Surgery for women with pelvic organ prolapse with or without stress urinary incontinence

Abstract: In women with POP and SUI (symptomatic or occult), a concurrent MUS probably reduces postoperative SUI and should be discussed in counselling. It might be feasible to postpone the MUS and perform a delayed (two-stage) continence procedure, if required.Although an abdominal continence procedure (Burch colposuspension) during abdominal POP surgery in continent women reduced de novo SUI rates in one underpowered trial, another RCT reported conflicting results. Adding an MUS during vaginal POP repair might reduce … Show more

“…The authors speculated that extensive traction on the bladder neck made by ASC mesh and Burch colposuspension could be the reason of the failures and de novo urgency. On the contrary, little or no effect of Burch colposuspension on postoperative failures, de novo overactive bladder, or voiding dysfunction was demonstrated in a recent Cochrane review (31). In our study, four out of 43 patients underwent Burch colposuspension for SUI with RFC for vaginal vault suspension, and surgical success was obtained in three out of four patients at 16 months, which was coherent with the surgical success rate of Burch colposuspension group.…”

“…The LBC is an ideal procedure to perform during concomitant laparoscopy for repair of prolapse or other indications. However, the value of a concomitant Burch colposuspension at the time of laparoscopic pelvic floor repair has been questioned in a recent Cochrane review, suggesting little or no effect on post‐operative SUI, de novo OAB and voiding dysfunction …”

“…The majority of women with POP and urinary incontinence have symptoms of mixed urinary incontinence 1 . These disorders may improve or resolve after POP surgery, with the level of success depending on patient characteristics and type of surgery 2,3 . The cough stress test (CST) is recommended in the evaluation of female patients to identify the signs of SUI and is used as an outcome measure following SUI treatment.…”
