“…Despite the development of improved operative techniques and postoperative care, emergent open surgical repair of acute descending aortic disease is still associated with significant mortality and morbidity. In the largest published surgical series, mortality rates ranged from 15 to 28% for acute traumatic rupture [7–10] and from 50 to 60% for ruptured thoracic aneurysm or aortic type B dissection [11–13]. Moreover, paraplegia continues to be a catastrophic complication associated with thoracic aortic surgery [7–9].…”

“…5) are excluded and typical symptoms are present, other forms of dissection, such as class 3 and 4 should be considered. Plaque ulceration following plaque rupture is typically visualized by TEE, CT or MRI [105][106][107][108][109][110]135,[139][140][141] . It has to be taken into account that multiple lesions are often present.…”

“…Moreover, surgical repair of the descending thoracic aorta is frequently complicated by respiratory disease, renal insufficiency or spinal ischemia (in 20%, 14%, and 9% of cases respectively) [6][7][8][9][10]. The early surgical mortality rate ranges from 9% to 38% [6][7][8][9][10][11][12] and is probably underestimated for elderly and atherosclerotic patients with a penetrating aortic ulcer.…”
