Suprapatellar versus infrapatellar approach for intramedullary nail fixation of tibial shaft fractures: a review of the literature

Abstract: Semi-extended suprapatellar intramedullary nail fixation of tibial fractures has recently been gaining popularity. Several recent studies and meta-analyses compare the outcomes of the suprapatellar approach with the traditional infrapatellar approach. “…Due to the long fracture line of the tibia, traditional internal xation with plates exposes a large area of the operative eld and requires dissection of the periosteum, which damages the soft tissues and blood ow, making the healing time of the fracture longer and increasing the risk of postoperative wound infection and non-union of the fracture [6][7][8] . Compared with traditional plate internal xation, intramedullary nailing is less demanding on soft tissues, more securely xed, less likely to displace the fracture, and can avoid secondary injuries caused by repeated operations due to poor plate xation [9][10][11] .
