Supraglottic jet oxygenation and ventilation assisted fiberoptic intubation in a paralyzed patient with morbid obesity and obstructive sleep apnea: a case report

Abstract: BackgroundHypoxia is a major concern and cause of morbidity or mortality during tracheal intubation after anesthesia induction in a pathological obese patient with obstructive sleep apnea (OSA). We introduce a case using Supraglottic jet oxygenation and ventilation (SJOV) to promote oxygenation/ventilation during fiberoptic intubation in a paralyzed patient with morbid obesity and OSA.Case presentationA 46-year-old man weighting 176 kg with BMI 53.7 kg/m2 was scheduled for gastric volume reduction surgery to r… Show more

“…The intermittent powerful jet pulses are able to reduce CO 2 washout and CO 2 dilution 33 from the jet flow, and provide acute measurement of P ET CO 2 , as demonstrated by previous studies. 23 , 34 36 Respiratory depression or apnoea can be detected effectively, while simultaneously monitoring the effectiveness of SJOV during moderate-to-deep sedation. Additional airway assistance may be provided to prevent the occurrence of hypoxaemia and accumulation of CO 2 during SJOV.…”

“…Given the deeper sedation level and higher altitude, we assumed that 30% of patients in the nasal cannula oxygen supply group might encounter hypoxia, which was still within the reported range [ 12 , 14 ] and consistent with our unpublished preliminary data. Considering the ventilation competence of SJOV [ 5 , 11 , 16 ], the incidence of hypoxia might not deviate from the reported rate (3%). We assumed an incidence of 5% in the SJOV group, which corresponded to an absolute difference of 25% between the groups.…”

“…In the SJOV group, SJOV was conducted using a Wei nasal jet tube (WNJ, Well Lead Medical Co. Ltd., Guangzhou, China), which was connected to a manual jet ventilator (Well Lead Medical Co. Ltd., Guangzhou, China) via its jet port (Fig. 1 ) [ 8 , 11 ]. The initial settings of SJOV were as follows: driving pressure (DP) 15 psi; respiratory rate (RR) 20 bpm; inspiratory-to-expiratory (I/E) ratio 1:2, and gas supply, 100% oxygen [ 8 ].…”

“…Due to its comfort and simplicity of wearing, endoscopic face masks are more widely used in clinical practice. The Wei Nasal Jet ® (WNJ) tube is a newly invented 2-wall nasal airway for supraglottic jet oxygenation and ventilation (SJOV) and end-tidal partial pressure of carbon dioxide (CO 2 ) monitoring [9][10][11][12]. Its inner diameter is 5.0 mm, outer diameter is 7.5 mm, and length is 18 cm.…”

“…SJOV is a minimally invasive technique of jet ventilation with the jet nozzle above the vocal cords and delivering high concentrations of oxygen with jet ventilation driving pressure [10]. It aims at improving oxygenation in patients with respiratory depression and is widely used in cases with difficult airways [6,[10][11][12][13][14][15][16][17].…”
