Background: Nonoperating room anesthesia during gastroenterological procedures is a growing field in anesthetic practice. While the numbers of patients with severe comorbidities are rising constantly, gastrointestinal endoscopic interventions are moving closer to minimally invasive endoscopic surgery. The LMA Gastro™ is a new supraglottic airway device, developed specifically for upper gastrointestinal endoscopy and interventions. The aim of this study was to evaluate the feasibility of LMA Gastro™ in patients with ASA physical status ≥3 undergoing advanced endoscopic procedures. Methods: We analyzed data from 214 patients retrospectively who received anesthesia for gastroenterological interventions. Inclusion criteria were upper gastrointestinal endoscopic interventions, airway management with LMA Gastro™ and ASA status ≥3. The primary outcome measure was successful use of LMA Gastro™ for airway management and endoscopic intervention. Results: Thirtyone patients with ASA physical status ≥3, undergoing complex and prolonged upper gastrointestinal endoscopic procedures were included. There were 7 endoscopic retrograde cholangiopancreatographies, 7 peroral endoscopic myotomies, 5 percutaneous endoscopic gastrostomies and 12 other complex procedures (e.g. endoscopic submucosal dissection, esophageal stent placement etc.). Of these, 27 patients were managed successfully using the LMA Gastro™. Placement of the LMA Gastro™ was reported as easy. Positive pressure ventilation was performed without difficulty. The feasibility of the LMA Gastro™ for endoscopic intervention was rated excellent by the endoscopists. In four patients, placement or ventilation with LMA Gastro™ was not possible. Conclusions: We demonstrated the feasibility of the LMA Gastro™ during general anesthesia for advanced endoscopic procedures in high-risk patients.

Laryngeal mask versus facemask in the respiratory management during catheter ablation

Background: The purpose of this study is to investigate if a laryngeal mask could improve respiratory condition during radiofrequency catheter ablation (RFCA). Methods: Twenty-four consecutive patients who underwent RFCA for atrial fibrillation were divided into two groups (Facemask group; n = 10, Laryngeal mask group; n = 14). All patients were completely sedated under intravenous anesthesia and fitted with artificial respirators during the RFCA. The capnography waveforms and their differential coefficients were analyzed to evaluate the changes of end-tidal CO 2 (ETCO 2) values, respiratory intervals, expiratory durations, and inspiratory durations. Results: During the RFCA, ETCO 2 values of the laryngeal mask group were higher than those of the facemask group (36.0 vs. 29.2 mmHg, p = 0.005). The respiratory interval was significantly longer in the laryngeal mask group than those in the facemask group (4.28 s vs.5.25 s, p < 0.001). In both expiratory and inspiratory phases, the mean of the maximum and minimum values of CO 2 was significantly higher when using a laryngeal mask than when using a facemask. The inspiratory-expiratory ratio of the laryngeal mask group was significantly larger than that of the facemask group (1.59 vs. 1.27, p < 0.001). The total procedure duration, fluoroscopic duration and the ablation energy were significantly lower in the laryngeal mask group than in the facemask group. The ETCO 2 value is the most influential parameter on the fluoroscopic duration during the RFCA procedure (β = − 0.477, p = 0.029). Conclusions: The use of a laryngeal mask could stabilize respiration during intravenous anesthesia, which could improve the efficiency of RFCA.

ÖZ GİRİŞTravma, dünyadaki her 10 ölümün yaklaşık 1' ine neden olan önemli ve uluslararası bir sorundur(1) . Bü-yük ölçüde genç nüfusu etkiler. On beş-elli dokuz yaş grubunda, tüm ölümlerin %13-29' undan sorumludur. Travmada ölüm genellikle (%20-86) hastane öncesi ortamda meydana gelir (2) . Özellikle erken ve etkili güvenli hava yolu desteğinin, travmazedelerin yaşamda kalmasını artırabileceğine yönelik çok sayıda teorik ve niceliksel kanıt bulunmaktadır. Hastaneye varmış politravma hastalarının morbidite ve mortalitesi genellikle hava yolu yönetimindeki hatalar sonucu ortaya çıkar. Gruen ve ark.(3) hastaneye ulaştırıl-mış 2594 politravma hastasında mortaliteye katkıda bulunan hata modellerini saptamaya çalışmışlar, hava yolu güvenliğinin sağlanamaması veya korunamamasının mortaliteye ilişkin en yaygın faktör olduğunu bulmuşlardır.Günümüzde, fasiyal travmalar, trafik kazaları, endüstriyel kazalar, silah ve bıçak saldırıları, terör eylemleri ve savaş yaralanmaları nedeniyle oldukça yaygındır (4,5) . Fasiyal yaralanmalar, kafa travması ve hava yolu bütünlüğünde bozulma ile birlikte değilse genellikle yaşamı tehdit etmez. Bu tür hastalarda hava yolu güvenliği ile ilgili başlıca riskler; hastanın hava yolu anatomisinde bulunan kemiklerin parçalanması, yumuşak doku bütünlüğünün bozulması ve aspirasyon pnömonisi insidansının artmasıdır (6) . Travma yönetiminin ABC'lerinin acil serviste yapılmasını sağlamak için ciddi bir fasiyal travma ile uğraşırken multidisipliner yaklaşıma gereksinim duyulmaktadır. Amaç; hava yolu güvenliğinin sağlanması, hastanın solunumunun stabilize edilmesi, kanamanın kontrol edilmesi ve servikal vertebralarda herhangi bir hasar olmadığından emin olmaktır (7) .Supraklavikular bölgede yaralanması olan bir hastada görüntüleme ile aksi ispat edilene kadar servikal vertebra hasarı var olarak kabul edilir (8) .
