Supracricoid Partial Laryngectomy for Primary and Recurrent Laryngeal Cancer

Abstract: This series demonstrates excellent local control for both primary and recurrent laryngeal cancers, with functional larynx preservation. In appropriately staged and selected patients with T2 or T3 primary laryngeal cancer or laryngeal cancer following prior radiation treatment, SCPL should be considered as a treatment alternative to non-surgical treatment or total laryngectomy.

“…The addition of postoperative radiation therapy to a specialized surgical organ-preservation procedure has been thought to compromise anticipated functional outcomes, thus discouraging studies on the combination of surgical and non-surgical treatments. 5 Although the nature and dosage of pre-operative treatment certainly plays a key role, comparisons between the study of Dufour et al (2004) 13 and Succo et al (2015) 15 seem to support that apart from the increased appearance of various side-effects, survival rates are also significantly compromised. Thus, the use of induction chemotherapy before surgical treatment seems to be contra-indicated.…”

“…Some reports show similar survival but better voice outcomes for such treatments compared with total laryngectomy (TL). [21][22][23][24] Although some studies have examined voice or swallowing function after nonsurgical treatment, 9,25-29 few have contrasted the nonsurgical patients with surgically treated patients, and even fewer studies have assessed voice or swallowing function more than 2 years after treatment. The purpose of the present study was to compare survival, recurrence, larynx preservation, and long-term voice and swallowing function in patients who received surgical vs nonsurgical therapy for advanced supraglottic laryngeal cancer.…”
