Supracricoid Laryngectomy with CHEP: Functional Results and Outcome

Abstract: This technique is useful in the treatment of selected cases of T3/T2 glottic cancer regarding the extent of disease. The incidence of complications in need of a complete laryngectomy does not compromise the functionality of this technique. The survival is comparable to patients who submitted to total laryngectomy and near-total laryngectomy, regarding the extent of the lesion.

“…To our knowledge, this is the first report of a detailed functional evaluation in an aged population after SCL. The functional results after SCL have been reported by different authors around the world 1–9, 23–29. In all the series, patients aged over 65 years, frequently up to the late 70s, have been included, suggesting that SCL are routinely performed also on old patients.…”

“…The results of this study support the findings of other investigators that SCPL initially results in severe swallowing dysfunction, most notably aspiration, but permits the eventual return to oral nutrition for most patients. [5][6][7][8][9][10] Unlike other stud-ies, however, this investigation retrospectively analyzed course of recovery and swallowing outcomes on the basis of objective instrumental analysis. To our knowledge, this is the first detailed scientific examination that delineates the specific postoperative swallowing physiology and symptoms associated with surgical defects after SCPL.…”

“…Similar to the findings of other investigations, this study confirms the high incidence of severe aspiration in the immediate postoperative period following SCPL. 8,10,14 In our institution, patients generally begin to eat orally despite their aspiration but are closely monitored. However, we rely on the findings from instrumental testing, specifically, the MBS study to help determine patient readiness for oral intake.…”

“…All patients are decanulated after surgery and present an intelligible voice. However, residual dysphonia is always present due to major larynx architectural modifications 4–6. The voice generally improves after a long rehabilitation period, but it is currently impossible to predict the final voice quality.…”
