Superior mesenteric–portal vein resection during laparoscopic pancreatoduodenectomy

Abstract: In our experience, LPD with concomitant major venous resection is feasible even in cases of longitudinal venous invasion. Further studies are needed to evaluate the role of laparoscopy in borderline pancreatic cancer.

“…For example, the non-specific term "hybrid" has been used to describe surgeries where resection is performed through laparoscopic approach and reconstruction is performed through a mini-laparotomy, 7 under robot assistance, 8,9 or with hand-assistance. 10 Terms such as "totally laparoscopic," "pure laparoscopic," "completely laparoscopic" versus simply using the term "laparoscopic" 11 to describe the procedure make it challenging to navigate the literature. Other terms such as "hybrid" and "minilaparotomy" are vague and represent different approaches depending on the study.…”

“…Indications and contraindications to LPD also depend on the experience of the surgical team [12]; with increased experience, it may also be performed for the treatment of tumors involving surrounding organs or vascular structures, and almost all contraindications to LPD may become relative. In this scenario, some pioneering groups have also started performing venous resections during LPDs [7,8].…”

“…This slow diffusion can be explained by three main reasons. The first one is the technical complexity of LPD, especially due to the retroperitoneal position of the pancreas and the proximity to the duodenum and surrounding vascular structures; the fashioning of the laparoscopic anastomoses; and the laparoscopic dissection of the uncinate process from the large vessels [6][7][8].…”

“…Evidence remains scarce regarding the role of MIPD in the setting of vascular resection for tumor invasion. Only a few case series from expert centers have been published on MIPD with vascular resection and reconstruction [49][50][51][52]. These mostly include venous resections and report acceptable perioperative outcomes.…”
