Superior and Inferior Ophthalmic Vein Thrombosis in the Setting of Lung Cancer

Abstract: Superior ophthalmic vein thrombosis is extremely rare and is often associated with orbital inflammation/infection, systemic/local tumors, hypercoagulable states, autoimmune conditions, and rarely carotid cavernous fistula. Clinical features include abrupt onset of painful proptosis, chemosis, ophthalmoplegia, and diminished visual acuity. Prompt diagnosis and treatment are essential to prevent permanent blindness. Management options for this medical emergency include antibiotics, steroids, and/or anticoagulati… Show more

“…The SOVT in SLE without antiphospholipid syndrome is attributed to inflammation and SOVT in SLE with the antiphospholipid syndrome is attributed to hypercoagulation [[13], [14], [15]]. There is an association between the systemic malignancies (leukemia, lymphoma, multiple myeloma, lung cancer and amyloidosis) and the SOVT [[3], [4], [5], [6],16,17]. We also detected a case of breast cancer with SOVT although the patient was complicated by cellulitis.…”

“…Además de las enfermedades orbitales y del seno cavernoso, hay factores de riesgo sistémicos, como estados de hipercoagulabilidad o amiloidosis, así como enfermedades inflamatorias y malignas que pueden predisponer a la trombosis de la vena oftálmica superior 12,[18][19][20][21] . Otros elementos, como los que componen la tríada de Virchow (estasis vascular, daño endotelial e hipercoagulabilidad), desempeñan un papel importante en la fisiopatología 19 .…”

“…Functional resistance to APC-R assays and genetic testing using DNA-based techniques can both be used to identify the FVL mutation and differentiate between heterozygotes and homozygotes. In the Caucasian community, the FVL mutation was discovered to be the most prevalent inherited thrombophilic condition, accounting for up to 37% of venous thrombosis cases [ 17 ]. In our case, ophthalmic vein thrombosis may be a consequence of the synergism between the two hematological abnormalities.…”

“…It is important to rule out an infectious cause (that would require antibiotic treatment). In aseptic cases, anticoagulant treatment can be initiated, but only after an adequate assessment of the associated bleeding risks [ 17 ]. Additionally, treatment with corticosteroids can be useful in reducing orbital inflammation and congestion [ 22 ].…”
