18F-Fluorocholine PET/MRI in Restaging of Prostatic Carcinoma in Relation to PSA Level and Detection of Active Disease

Abstract: F-FCH-PET/MRI using an integrated system with 1,25 MBq/kg dosing of FCH is a valuable tool in evaluation of restaging in patients with prostatic carcinoma, with high detection rate even in those with a low serum PSA level.

“…A meta-analysis was performed on 23 studies (Fig. 1), 11 concerning the staging phase [16,18,27,35,38,40,[44][45][46][47]52], and 12 the restaging phase [8,21,23,25,26,30,32,39,42,51,56,59]. Pooled sensitivities and specificities were obtained for the former (staging), and a pooled detection rate was computed for the latter (restaging).…”

“…PET/MRI was used to seek biochemical recurrences of PCa in a total of 598 patients [8,26,32,34,39,42,51,52,56]. Taking the studies concerned together, the recurrent disease detection rate achieved with PET/MRI ranged between 54.5 [56] and 97% [8] (Table 2).…”

“…One of the largest recent studies carried out by PROMETHEUS, a Multicentric European Study, confirmed PHI as one of the strongest predictors of PCa, correlating with the Gleason Score (GS). In our own studies, firstly monocentric [5,8] and later on multicentric (with our partners) [9,10], we proved the PHI's ability to distinguish between PCa GS < 7 (low aggressiveness) and GS ≥ 7 (higher aggressiveness).…”

“…Previous publications reported on overall detection rates for FCH or 11C-choline PET/MRI in recurrent PCa [2,[16][17][18]. For instance, Eiber et al [16] found an overall detection rate of 84% in 75 patients with median PSA levels of 2.6 ng/mL (range 0.2-88 ng/mL).…”
