SUNCT: bilateral hypothalamic activation during headache attacks and resolving of symptoms after trigeminal decompression

Abstract: Short-lasting unilateral neuralgiform headache with conjunctival injection and tearing (SUNCT) is a primary head-pain syndrome, which is often refractory to any medical treatment. Concerning the pathophysiology of SUNCT, hypothalamic involvement ipsilaterally to the pain has been suggested based on the clinical features and one functional imaging case report. Here we now report a new case with SUNCT and the concomitant cerebral activation pattern (fMRI) during the pain attacks. In addition to an activation of … Show more

“…This supported the notion of MVD being a potential treatment for these conditions. To date, 10 case reports and a case series of nine patients with medically intractable SUNCT and SUNA patients, who underwent MVD of the trigeminal nerve, have been reported [26,[30][31][32][33][34][35]. After a median follow-up of 14 months (range 0.5-32 months), 12 of 19 (63 %) of cases were pain free, whereas in the remaining patients, the procedure had little or no effect.…”

“…However, some unusual features are present. The age at onset usually is older than in this patient: 50.7±14.8 years in the Pareja and Sjaastad review [4], but juvenile and even childhood onset have been described [10,11]. In the IHS Classification [2] the frequency of attacks is from 3 to 200 daily, but in their review Pareja and Sjaastad [4] report a greater variability: from less than 1 attack daily to more than 30 attacks per hour and the usual frequency ranged from 1-2 to 20-80 attacks per day.…”

“…No SUNCT foi descrita uma ativação da área postero-inferior do hipotálamo ipsilateral a dor 19 , mesma alteração encontrada na cefaléia em salvas 20 , bem como uma ativação hipotalâmica bilateral 21 . Esses achados falam a favor de uma origem central para esta cefaléia.…”
