Summary of Results Using the Nucleus CI24M Implant to Record the Electrically Evoked Compound Action Potential

Abstract: Using the protocol outlined in this study, it is possible to reliably record EAP responses from most subjects and for most electrodes in Nucleus CI24M cochlear implant users. These responses are robust and recording these responses does not require that the subject sleep or remain still. Based on these results, a specific protocol is proposed for measurement of the EAP using the NRT system of the CI24M cochlear implant. Potential clinical implications of these results are discussed.

“…The parameters that were applied in recording the neural response were taken from published recommendations [14][15][16] . ECAP threshold measurements were taken at the 80 and 35Hz stimulation frequencies to assess possible changes in the characteristics of the neural response as the stimulation frequency is reduced.…”

“…Telemetry also supported the world's first recording of the ECAP using the intracochlear electrodes via NRT. 21 Neural response telemetry can be used to assist in device fitting [22][23][24] and can be very helpful in fitting young children 25 and others who do not give reliable behavioral responses. It has been shown that NRT-based fitting procedures are quicker than behaviorally based methods in young children, with similar aided thresholds and speech detection scores for the 2 methods.…”

“…In case the action potential could not be observed in any of these electrodes, other adjacent pairs were used as active and reference electrodes for the optimization series, until some positive response could be observed. We considered it absence of intraoperative NRT when we did not observe ECAP in any of the tested electrode pairs, considering as bases the characteristics described by Abbas et al 15 . The maximum intensity used in the test was of 230 current levels.…”

“…Obtaining data related to the cochlear nerve's permeability towards electrical stimulation and the way through which some electrical stimulation parameters interact with remaining neural structures still constitutes a challenge. Early determination of exciting neural elements present in the different cochlear pathological processes would also be very useful, since it is supposed that the survival of ganglion cells and other neural elements may constitute one of the causes for variability in relation to speech recognition performance found in implanted individuals 3,15 .…”
