Successful vascular decompression in an 11-year-old patient with trigeminal neuralgia

Abstract: Trigeminal neuralgia in pediatric patients is very rare. We present a case of typical trigeminal neuralgia in a child, demonstrating the pathogenesis of the neurovascular conflict due to subarachnoidal adhesions after meningoencephalitis.

“…18 In the pediatric population, Solth found success utilizing MVD in an 11-year-old patient, whose symptoms were caused by nerve compression via the superior cerebellar artery and anterior inferior cerebellar artery. 31 In a study of female pediatric patients at John Hopkins, 5 of the 6 cases also experienced complete pain relief via MVD. 16 Complications that have been associated with MVD include CSF leaks,…”

“…Of 52 pediatric patients among 9 series, complete response to MVD was observed in 33 cases while partial response was seen in 17 cases at follow-up times ranging from 6 months to 18 years. 4,12,15,16,19,20,23,28,31 Other surgical procedures used in the treatment of pediatric trigeminal neuralgia include percutaneous balloon compression, and peripheral glycerol injection. 19 The percutaneous balloon compression performed by Baabor demonstrated high success rate, low recurrence rate, low cost, and a low morbidity rate.…”

“…Recently, it was proposed that the cause of trigeminal neuralgia is herpes simplex 3 . Solth et al 13 described a case during the intraoperative period with severely thickened fibrous tissue in the pontomesencephalic cistern which attached the deviating blood vessel to the trigeminal nerve. This tissue, which was neither of arachnoidal nor of neuro-ectodermal origin, could be scar tissue from the severe Epstein-Barr virus infection which this child had contracted at the age of 3 years.…”

“…19 Solth A reported a case of TGN in a 11 year old boy who had meningoencephalitis. 20 Trigeminal neuralgia has also been associated with lipomas in children. 21 Glossopharyngeal neuralgia arises due to various causes such as tumor , infection , Chiari I malformation, infarction, dissection of the vertebral artery or neurovascular compression.…”
