Successful treatment with IVIg therapy of diabetes-associated scleredema severe progressive case and review of the literature

Abstract: Scleredema is a deposition disorder which presents as diffuse, symmetric, non-pitting skin induration from dermal deposits of glycosaminoglycans ("mucins"). It classically affects the upper back and posterior neck, often causing skin tightness and decreased range of motion. In most patients the clinical course is chronic and treatment options are limited. We report a case of a patient diagnosed with scleredema associated with longstanding insulin dependent diabetes who was treated successfully with IVIg. Addit… Show more

“…IVIG was used in the treatment of 10 patients with severe and resistant pyoderma gangrenosum who could not respond despite multidrug therapy, and improved in seven patients (Cummins, Anhalt, Monahan, & Meyerle, 2007). In treatment-resistant livedoid vasculopathy and scleredema, it has been reported that IVIG is fast effective and safe treatment option (Kennemer, Pavlidakey, & Sami, 2017;Yoshioka, Tateishi, Kato, & Chen, 2018). (Fernandez & Kerdel, 2007;Forbat et al, 2018;Smith, Dennington, & Cooper, 2010).…”

“…NB-UVB phototherapy may affect keratinocyte-fibroblast interaction to inhibit fibroblast activation 3 . IVIG has been reported with successful response in some cases 4 5 . It is suggested that it may reduce glycosaminoglycans in the skin by acting on the Fc receptor and regulating innate and adaptive immunity.…”
