Successful Treatment of Severe Lactic Acidosis Caused by a Suicide Attempt with a Metformin Overdose

Abstract: Metformin-associated lactic acidosis is a very rare but critical condition. It is seen in patients with type 2 diabetes mellitus who take metformin and attempt suicide with a metformin overdose. Here, we report a 43-year-old woman with type 2 diabetes mellitus and chronic renal insufficiency who developed hypoglycemia, hypothermia, tachycardia and lactic acidosis after a suicide attempt with a metformin overdose. She was successfully treated by continuous venovenous hemofiltration, and adequate hemodynamic and… Show more

“…An acute overdose of metformin can cause a severe lactic acidosis [48][49][50][51]. In such cases, a depressed level of consciousness may occur following several hours of lucidity, although typically it is in the setting of cardiovascular instability [47,[52][53][54][55][56]. Hypothermia, as observed in our patient, has also been well described in acute metformin overdoses [47,54,[57][58][59][60].…”

Similar to ethylene glycol, at most institutions including our own, metformin measurements are send out tests, with results being available days after empiric clinical management decisions are necessary.…”

“…[3536] Metformin has been used in massive quantities with an intent to self-harm in a few patients. [4647484950] Use of excess doses of metformin results in lactic acidosis which may result in cardiac arrest[4750] and even death. [50] Hemodialysis or hemofiltration have been used to correct lactic acidosis in these situations.…”

“…9 Recently, Yang et al reported a 43-year-old woman with type 2 diabetes mellitus and chronic renal insufficiency who developed hypoglycemia, hypothermia, tachycardia and lactic acidosis after a suicide attempt with a metformin overdose. 10 They pointed out that early diagnosis and rapid correction of the metabolic acidosis using hemodialysis or hemofiltration, together with concomitant cardiovascular support, and maintenance of blood glucose and core body temperature, provide the possibility of a positive outcome.…”
