Successful Treatment of Early Endometrial Carcinoma by Local Delivery of Levonorgestrel: A Case Report

Abstract: We describe a case of a 67-year-old Caucasian woman with an early, moderately-differentiated adenocarcinoma of the endometrium. A levonorgestrel-releasing intrauterine system was inserted, which she tolerated well. A full D&C, following removal of the device, was performed after 9 months, confirming absence of tumoral tissue. Examination after 24 months showed a very thin endometrium, indicating complete remission.

[74][75][76][77] Another benefit is the usefulness of the LNG-IUS in women with adenomyosis and fibromyoma associated HMB. 78,79 Furthermore, the combination of intrauterine progestogen delivery to suppress the endometrium, in combination with systemic estrogen, is highly appreciated by women resulting in a high continuation of use due to the absence of side effects and erratic bleeding in the large majority of women.…”

“…[45] Later, several reports followed using the framed Femilis ® (Contrel Research) LNG-IUS and included two cases of endometrial carcinoma, all successfully managed. [46,47] No failures occurred during follow-up for many years. The numbers were small but indicative of the extraordinary impact of LNG to suppress the hyperplastic and atypical endometrium.…”

“…An outpatient endometrial pipelle biopsy was performed which revealed a moderately differentiated adenocarcinoma ( Figure 7) with minimal myometrial invasion (Clinical FIGO stage I). 35 An LNG-IUS (Femilis ® ) was inserted and advice was given to conduct a repeat biopsy within the next 3 to 6 months. Spotting continued for several weeks and then stopped.…”
